  • In this film, the GP is seeing a patient who has presented several times before with complaints of palpitations and shortness of breath.
    The patient describes the onset of her problems, which was a panic attack. She has then gone on to develop agoraphobia. The problem has affected a number of areas of her life.
    The GP explores the patient's understanding of the problem and it is apparent that she has attributed this to a problem with her heart. The GP then goes on to give the patient an alternative explanation; that this is anxiety. She describes the physiological symptoms of anxiety and explains why avoidance is a problem.
    Please note that this video has been made by the University of Nottingham for teaching purposes. The psychiatrist is a real psychiatrist but the patient is played by an actor.

    Note at 3:40 she mentions she is becoming afraid to go out. Possibly the beginning of agoraphobia, an all too common and debilitating comorbidity with panic disorder.

      @@oppressednolonger1497 as someone with GAD and Agoraphobia, I can honestly say, it's very hard to treat. You have to get out of the house and out of your comfort zone, and as an adult, it's very hard to agree to that knowing that the amount of anxiety you'll have to wade through to feel better is enormous. It certainly does and can get treated, but it's hard. Patients don't always cooperate. I know I don't. I know I NEED to and that I'll be happier and less depressed, but it's very very scary. I tend to have panic attacks just leaving.

    This Doctor has a wonderful way of engaging with patients, she asks many open questions, she reflects back to check understanding, she uses the patients language, and is very approachable and understanding. I am a medical student studying for exams and these vids have been very helpful bravo!

      @@wyleFTW Real GP and an actor. Probably a real case, though. Using a real patient could be unethical and unpredictable. Some instructional videos have real patients participating, though. Like Neuro PACES series.

    I was at 18 years old when I came to my doctor for the same issue (also anxiety induced) and I remember him sitting me down in one of the exam rooms and just looking in the eye and telling me "you don't have to live like this" and that's when I started treatment.

    Wow! 🤯 what an amazing doctor! I wish she was my doctor. I was diagnosed with GAD 6 months ago (I am now fully recovered) and saw 2 therapists and they never gave me an explanation like this! I now fully understand! Im sooo glad I stumbled across this video 🙏🏼

    I did got an ECG at first! Came back perfect. Anxiety is such a trickster, it will make you believe and doubting anything.

      Same. At age 7, I saw a movie in which a child had to have open heart surgery and it flipped a switch in my head and I couldn't stop thinking about how children like myself could die. I started having panic attacks and they sent me to a cardiologist, which revealed no issues, so then they sent me to a psychiatrist. It's such an exhausting condition.

    People saying that the Dr says uh hm, yes and ok too much I think she is trying to show empathy and show her that she is following along with her or that she understands what is being said so Julie doesn't feel misunderstood or crazy. Either that or she is nervous. She might not realise and she's taking it too far. I think people would be surprised how much they do something similar in every day situations, but those don't get recorded for training purposes like this one here.

    I’ve suffered from anxiety as a kid and I’m 30 now, mine was the stomach aches and the fear. Eventually at 28 I panicked. I actually learned something from this video for myself and others.

    Agreed: this is Anxiety. Anxiety causing- just by watching or listening. If this kind of interruption and leading approach works for you, great. If it doesn’t- don’t “panic”. Some great therapists offer a platform for space between words and actual unwinding of thoughts or breath. Also- considering the issue- an obvious resolution would be to ask the client to untie the bow that’s restricting her diaphragm. That kind of pressure most always causes anxiety.

    I wish the counselors in the US were more like this. Instead of suggesting CBT or something, they should take the time to explain to you what's going on and how you can fix it.

    The client seems so genuine and vulnerable. Personally I'd feel much more at ease in her position if the doctor mimiced my gentle tone and overall vibe. Enough with the rushing of questions and the "right" fillers.

    as well I have panic disorder. Attacks started when I was a child. Nobody understand what is that. They thought it will pass but not. Right now I have 31 years old. It had been chronical unfortunately.

    4 ecgs 2 echo test 1 trade mill test 1 hotler ecg 24 h control for blood pressure 1 ct angiography with 0 calcium score no narrowing no plaque for 2 last year and yet constant chest pressure some time pain along left arm.
    Its like a hell it took a lot a courage and convincing to calm down.

    are GPs behaving like this lady in real time? My experience is that GPs are in hurry and force patients to stop talking. One reason might be short consultation time.

      Yeah unfortunately GPs only get 10 mins to do history exam and come up with management plan. This is 12 mins only on history. If this was a real situation I would want to check her pulse and do her BP and maybe listen to her heart to rule out cardiac cause as it could be palpitations not due to anxiety. So to do all of that is very hard in 10 mins! :)

      As you say 10 minutes is all you should get as consultation time with your GP however since recovering from encephalitis in 2013 my consultation with my GP has never been less than 30 minutes… I’m lucky because he originally trained as a neurologist … So therefore he understands very well how my brain is working… I struggle to understand what he’s saying to me he has to speak really slowly because of how encephalitis has affected me

    I have Generalized Anxiety Disorder with Bipolar Disorder and ADHD secondary to Sensory Processing Disorder. I also have a myoclonus that may be from my Chiari Malformation and possible Tethered Cord Syndrome that gets worse when I feel anxious (also when I'm in bright light). My Sensory Processing Disorder frequently causes me to be overstimulated by my environment, especially when there are a lot of people talking (sounds like a roar rather than individual voices), there are a lot of unexpected noises, noises that are unfamiliar to me, or people are unexpectedly touching me. Whenever this happens, I just think to myself, "okay, my muscles are jerking and shaking more than usual, my hands are sweaty, my chest feels tight, my heart feels like it's beating hard, and I'm breathing hard. I must be anxious about something. Am I actually in danger, or am I just overstimulated?" Most of the time, it's the latter, and, when that happens, I either just breathe through it and finish my business quickly, or, if I'm gonna be a while, I make a quick trip to the nearest bathroom to get out of that environment long enough to calm down.

    It is not by adrenaline, it is by cortisol response that cause panic and hypervigilance And with more acetylcholine domination makes the valves and artery constrict more !

    So, client gets a 10 min talk, after the talk the client is sent away with no solution whatsoever, just another appointment. No matter how well this doctor is "engaging", her result is zero. - What if the client gets another attack this afternoon, or every day from now on? Oh, she should just wait until her next 10 min talk.

    Not true that if you have a heart problem, th e ECG will pick this up. further tests should be carried out, at least a nuclear stress test, before rulling out heart problems. Specially if the patient has palpitation and chest pain.

      I'm just three years older than the patient in this video and don't smoke or drink have a healthy BMI am pretty active and just had a heart attack. I agree with you and I think it's irresponsible to automatically rule out heart problems just because this woman doesn't fall into the "risk category".

      I have a lot of these same symptoms this woman is mentioning and was told I have POTS finally this year after having it labeled “anxiety” for years. I also believe I have the type of POTS that the adrenal glands are involved in because my cortisol is high when It’s been tested and my blood pressure will go high often. I think for women especially it is sad how often health concerns get dismissed as anxiety and they have to suffer or wait years sometimes if ever to get an accurate diagnosis and help to manage so they can function and feel healthy again. Especially if that woman has had anxiety in the past, I have, and since it was in my medical records anything I felt was viewed as “anxiety”. It is truly sad.

    some comments are stupid, im sure this lady was not checked just once, she was back and fort thats why the doctor here showed up, instead of the usual doctor she usualy seek reassurance with. seeking reassurance for many many times making the anxiety worst, they becoming dependent in that kind of feeling of relief and cant help herself. the Word anxiety, wont be there if its not realy exist. and yes Diagnostic tests was very crucial, but? its not normal if youre the one whos forcing to have it instead of your doctor. and also ger doctor tells her shes fine, so what do you wany her to do? Huh? play doctor by herself? 😁😁😁 and still force the negative ones instead of the positive once? its not gonna cure her.

    I guess you're a student? Well then maybe you should know, there is some people for whom this kind of attitude is beyond annoying. When I'm going to the doctor I expect somebody to be concrete and not asking me questions without telling me why they need to know that and smiling at me all the time (the "crazy psychiatrist's grin") and being super sympathetic. It's so obviously false it turns me away immediately.

    why do psychiatrist think everyone else has a problem? why do they think every normal behaviour is abnormal and a mental disorder? a man can be sitting calmly and asking them in a perfectly normal manner “what injection is that that you are giving me?” and they can get hysterical accusing the man of being aggressive and violent when he is not.
    I think psychiatry itself is a mental illness. it is a mental illness of psychiatrists to see everyone else different from themselves as mentally ill. it is a mental illness of psychiatrists to want to control and manipulate people. I have seen many normal people entering psych wards because of their circumstances at home yet were all forced medications, after that they werent okay anymore. if psychiatrist are the ones always diagnosing people, who are the ones checking and diagnosing the psychiatrist?
