What are some instances of when studies and reports would not have the same n=x on the flow diagram? For example, would one report with a cohort and independent replication be considered to have two studies?
You may find an abstract from a conference presentation (report 1) as well as a journal publication (report 2) from the same study, which means you'll have two reports and one study. An independent replication on a different set of participants will be a study on its own. Enjoy the process. There are so much to learn out there.
You are asking a very relevant question. I don't have an official definition at hand, but I would think that something like Medline's Pubmed is a database and a study site's local archive of studies is a registry.

Prisma was developed within the health domain, however you can use the principles for all subject areas.. For studies related to health, we register the title of the systematic review through PROSPERO. However, for management studies there may be a different platform.
