The game was mainly in Japanese back then, but from what I remember Paracelsus had a much deeper tone of voice. I think the devs wanted him to sound a little more otherworldly, here he just sounds like a normal person.

Maybe you can't literally change your birth sex, but ultimately most things that define the genders, (clothing, beauty standards, social roles, ways of speaking etc.) actually have nothing to do with birth sex at all, like for example we think of women as not having any body hair, but most women do naturally and just shave it (I think at some point in arabic history a woman with a mustache was even considered beautiful). So somebody could (and many do) disregard these gender roles in their life and identify as not a boy or a girl (nonbinary)
Testament (the one in the right) is actually under the Non-binary umbrella (I think Agender is what would fit them best since theyre beyond the concept of gender and not in between) And uses they/them pronouns in this game

Testament is female, Bridget was raised to be a woman because lore tells reasons (she's a twin, it's bad omen and bad luck to have twins in her village).
Testament has no real gender so while appearing female in some games, their voice actor/actress varies from feminine to masculine depending on which language you have the game set to in order to show that they're technically neither. Personally prefer the masculine voice since it sounds so nice but the Japanese version has a feminine one. but yeah, officially since their introduction I believe, Testament has not been neither a man or a woman.
