As a frequent traveler to Thailand, I’ve taken both taxis and the airport link before. But mainly taxis, as I arrive from the U.S. with 2 checked baggage, a carryon and a backpack. In May, I took a taxi to Bangkok Sue railway station. The driver wanted 800 baht. I considered making him use the meter. I instead offered him 650 baht. I knew that the total fare would have been around 550 baht, with tolls and 50 baht airport surcharge and he would be loading and unloading my luggage. This October, I may try grab, or take airport link to Playa Thai and take a taxi from there. Trying to take the MRT blue line from Makkasan and the walk that involves would probably be too much after a 24 hour flight.
Yes, the prices are fixed and tolls to pay. Getting a taxi leaving Bang Saen is not easy on the other hand, so be aware of that. You will need to pre arrange a pickup or find a service while in Bang Saen to go back to Bangkok.
That size shouldn't cost you anything because it is below 26 inches. If the taxi driver insists, just pay him to avoid confrontation or use Grab. Most taxis drivers won't bother, but I have heard stories, so you never know. All the bag fees are posted near the taxi queue number dispensing machines.

The problem about taxi meter is drivers it not turn on meter right?
Usually not an issue at the airport, but yes in tourist areas or if there is very high dmand like when it is raining.

It varies a lot depending on the road and distance.
