Similar thing happened while I was serving in the military.
My supervisor ordered flat metal washers (for screws), thinking he was ordering a Unit of Issue (UI) of EACH; when in fact the UI was hundred (HD).
He ordered a quantity of 20, and ended up having 2,000 washers delivered to our workcenter...big oops!
