For Western Europeans I would recommend ordering one size smaller than their size, or check the second Russian measurement, for example, these boots in Russian size 40, have insoles which are 255 mm long. The boots indicade both sizing measures, 40 and 255. Just check that your feet are not longer than this second measurement.

Not really. Layer up on the socks and wear Moleskin, but you will get used to them. A very handy, and useful boot afterwards. Take a while to break in, I say give it 20 hours of wear before anything serious. I use them for farm work, shop work, reenactment, and sometimes daily wear. Really just wear them and you will find a groove.
I'm pretty sure those are post war boots, tovarisch. Wartime boots had the pimple pattern instead of the tire pattern on the soles.
