BED TO WHEELCHAIR TRANSFER
1. Perform hand hygiene. Introduce yourself and establish rapport. Explain the procedure.
2. Take the client’s baseline vital signs.
3. Attend patient’s personal needs before to transfer.
4. Lower the bed and ensure that brakes are applied. Place the wheelchair next to the bed at a
45-degree angle. If a patient has weakness on one side, position the wheelchair so that the
stronger side will come in contact first once the patient will stand.
5. Slide one arm under the person’s head and place palm flat on their back. Place your other arm
in their knees and grasp the back bottom of the patient’s knee then lift.
6. Apply proper footwear to the patient.
7. Properly put the gait belt on the patient.
8. Position yourself with one foot between the patient’s feet and one foot in the direction you will
be moving.
9. Be sure to tell the patient when you will begin the move so that s/he is prepared.
10. Do pivot if moving the patient to the wheelchair.
11. Place the footrests in the correct position.
12. Place a drape over the patient’s lap to provide privacy.
WHEELCHAIR TO BED TRANSFER
1. Establish rapport and explain the procedure to the patient.
2. Bring the wheelchair beside the bed. Make sure the person’s strong side is beside the bed.
Lock the brakes.
3. Move the footrests out of the way.
4. Wear the gait to the patient properly.
5. Position the patient with one foot between the patient’s feet and one foot in the direction you
will be moving.
6. Be sure to tell the patient when you will begin the move so that s/he is prepared.
7. Do pivoting if moving the patient to the side of the bed allowing the patient to sit properly at
the bedside.
8. Assist patient to assume sitting or lying position comfortably.
9. Obtain vital signs.
10. Hand washing.

Where is the seatbelt for the wheelchair!? Hello, huge safety violation. You cannot transport the patient around without the seatbelt applied to the client after the client is assisted into the back of the wheelchair. Not a good body mechanics technique as that can easily injure the staff that is transfering the pt. This opens the door to a twisting motion of the body and will result in lower back injury.

mali naman pag support mo sa weak leg nya. dapat ang right leg mo rin ang susupport sa weak leg para madali mag pivot. I'm a certified caregiver. support always the bad leg
