その通りです! 内頚動脈(ICA)閉塞でICA先端部が開通している場合、遠位塞栓させない血栓回収は難しいです。
吸引法では、この例のように遠位塞栓が起こります。たとえバルンガイドカテーテルで遮断し大口径の吸引カテ吸引しながら回収しても、血栓を取り残すかもしれません。
ステントリトリーバーが改良しているため、ステントで一体に回収する方が遠位塞栓を起こしにくいでしょう。
