Hi! I’m using it in a number of situations. My severe patients who have tried everything else are always wanting something new so a fair number of those patients have opted to try it. I do Schirmer tests on everyone in my dry eye clinic so if that’s low I’m considering it. I also have patients who are trying to stay away from out of pocket in office procedures and want something their insurance will help with so that is a factor as well. I would say so far the vast majority of my Rxs have been to patients in the moderate to severe range and most have mixed mgd/aq deficiency. Loved your video of you trying it! I haven’t seen a rep yet so my patients have tried it but I haven’t!

It does work pretty well. BUT IF one's insurance doesn't cover Travaya THE BASTARDS WILL charge you over $500 a month or you can go without. I have to Shout because nobody else is talking about this and the cost is not on the web. Did you even ask what they charged? Of course not, you have a business to run here and you are not helping out the little guy. I can afford it but I just don't know if I want to spend 6,000 a year. And it's not just sneezing, it's painful irritation.
A lot of people on Facebook dry eye groups talk that it helps them with oil production. Which makes sense because aqueous defficiency causes MGD. I always wonder if it's true that only 10% of dry eye is from lacrimal gland disfunction, I find it suspicious that a lot of doctors claim Schirmers tests are outdated when they can be the best test to determine MGD or aqueous deficiency.
I have a video coming soon on schirmers tests. I still use them for all dry eye patients. I find it’s very helpful. The mechanism of tyrvaya stimulating the trigeminal nerve means it will stimulate all tear production, increasing basal levels which yes - includes oil. I saw my first tyrvaya follow up yesterday and she went from schirmer OD 3mm OS 0 mm to OD 12mm OS 3 mm. Very encouraging!
@@jernejtkobal I’m not sure what the long term therapy will look like. My patient was concerned about the price and we talked about potentially reducing to once a day for maintenance. Time will tell I suppose
** BEWARE- I have severe dry eye and was prescribed this and after a week, while I had tears finally form in my eyes the SIDE EFFECT was VERTIGO and it took A MONTH to go away. I could NOT drive or stay focused at work. BE CAUTIOUS. I don’t wish vertigo aka severe dizziness on anyone.
I was on tyrvaya for a year, but then I started getting sinus headaches. I went off it for about 9 months. I think I was using it WAY too much. I started on it again a week ago. I'm now strictly limiting myself to two sprays per day (just sprayed as I realized it was time). About the time I went off it, my walking became kind of unbalanced. It's kind of a weird sensation. I'm not sure if it is from the tyrvaya or if it is an aging thing. I'm 63. I still have the weird walking/balance sensation. Are you still free of vertigo?
Good morning, I have tried the nasal spray. Yes, it is good. Idk if the fact I don’t normally sneeze, I haven’t sneezed in years. Idk why!! When I used the spray it made me sneeze like crazy at first. Seems to help. That’s one of my problems. When you talk about schirmer score, I was never told about a schirmer score so I do feel kinda out there. I have learned on my own about schirmer along with a lot of things. I would love to have a visit in person with you just to have some things explained. Hey, even the sneezing felt great!!! Kinda strange!! I was given a sample to try. I am seeing a local NEW opthamologist May 2nd. I’m truly looking forward to seeing you Dr. D. I cannot say it enough how thankful I am to have found you. I’m not giving up! I can’t and won’t. I’m just tired and frustrated. You have truly given me hope. You were telling my story when I first saw you. None of these videos seem to disclose the Psychiactric side effects of the medication which is very disturbing. Psychiatric Very common (10% or more): Insomnia (up to 19%), abnormal dreams (up to 13%), depressed mood disorders and disturbances (up to 11%), irritability (up to 11%) Common (1% to 10%): Agitation, anxiety, depressed mood, depression, hostility, middle insomnia, nervousness, nightmare, restlessness, sleep disorder, tension I'm not saying don't try the medication, but I feel people should be informed before deciding to.
Just got it today, my doctor said not to sniff it once you sprayed it but I don’t see anything in instructions saying you shouldn’t sniff it. Anyone knows?
Hi Dr, Appreciate your video, How accurate is a shirmer test? How can they know if its basal vs reflex tearing from the irriation of applying the strip. I once tested 1mm in each eye and the year before over 15mm which was obviously relex tearing. Does it have an instant effect or does it take 30 days to kick in? Thank you kindly
They're definitely not perfect. It depends on who is doing it. We use a topical anesthetic right before the procedure so that your cornea and conj are numbed and you don't produce the reflex tears. Also, have to dab at the anesthetic and get it out or else the whole test will show the proparicaine drops. Your test results would be suspicious to me, as that isn't normal to go from 1mm to 15mm without some serious intervention. Hope that helps!
@@talksick508 give it a go, got nothing to lose. I dont produce much tears myself. I see you dont either so ill share my routine which ive spent the last decade discovering through trial and error. Ive tried literally everything on the market (not exaggerating). If you havnt tried already get some polyvisc ointment to use before bed but also little top ups during the day. Polyvisc replaces the oil element but youl want a more watery drop to replace the aquaous layer too so something thin like thera tears preservative free is good to replace the aqueous layer. Adding thera tears after 15min or so after polyvisc or when you feel you need it. I also do heat compresses before bed with a microwavable eye mask then I use a device called nulids that expresses the oil after you heated it. Its a expensive device but does a great job of pushing oils out.Then some lid wipes usually with make up remover pads in warm water. Sleep with a night goggle that creates a moisture chamber. I use onyx goggles which are the best for that. Apply polyvisc to eyes before bed then wear onyx mask. Also krill oil or cod liver oil supplements help somewhat. During the day when outdoors I wear moisture chamber sunglasses by 7eye. Their a game changer to block wind and keep eyes moist. I also use them indoors if aircons on. Avoid aircon and heating as much as possible. Invest in a humidifer for your room to add humudity. Keep hydrated drink 1l of water every morning upon waking. Hopefully my routine helps you out. Try and keep positive mate.
I just started taking this, only on it for 2 days so far and haven’t noticed a big improvement yet but in your video you say you think a difference is first noticed after 2 weeks. I think the whole idea of certain treatments in this field having to "take some time" to see an effect is as well thought out scam. I was not impressed by tyrvaya
