Damn i am very very late but this song is actually very very good. All this feelings and emotions sound real and not fake. Comatose means "numb" when he was thinking of a song his producer said, "comatose" Because the song is about how him and mads broke up. The heart represents his broken heart. You can see in the 2nd seen that he has a blood spot where his heart is. Which case the heart represents his heart being broken/numb.
