I wonder, if these two characters are actually two sides of one personality, does it mean that Elisabeth herself when she was young was cruel and disrespectful towards the older actresses around her?
I'm pretty sure she was exactly like Sue. She only cared for good young healthy body. And no surprises she had no friends or desire to try something new.
Sue was basically the physical manifestation of Elizabeth’s inner insecurities about herself, she has the young, hot body that she so craves and at the same time the arrogant selfish attitude of her mind telling herself that she’s not good enough anymore

Elizabeth was stuck in a rut. If she gave up on The Substance, she'd be stuck with the scars left by her careless behavior. After being Sue, going back to Elizabeth was torture for her. Notice how sedentary she became as Elizabeth, she'd be waiting the whole week to go back to being Sue.

Yes like there is an addiction aspect to this movie. Amazing how it touches so many subjects at once. Fading beauty, sexualization, insecurity, addiction, misogyny, ageism, loneliness, fame hunger, superficiality,...
I think it's really elegant. The Substance is a trap. Theoretically you could maintain the experience indefinitely with no ill effect as long as you maintain the balance. And in theory the balance is fairly simple to maintain. But nobody would go through with the sketchy procedure unless they were truly desperate to become young and beautiful. The kinds of people who are not at all comfortable with whom they are, and who will inevitably lose balance.
