Not really. Even in microsurgical ligation or laparoscopic ligation, you think urologists ligate the external spermatic or dartos veins? The main culprit in varicocele is internal spermatic vein/s in most cases. Even with surgical failure and recurrence- venography identifies ISVn recanalization or collaterals in all cases and embolization is gold standard in such cases. So, the external spermatic veins is more of a theoretical proposition.

I had this procedure and it literally ruined my life. I’ve had debilitating chronic abdominal pain since, to the point that I can’t walk, climb the stairs, doing shopping. For the first time in my life I have my parents looking after me (I’m 36). Coils can create scar tissues and abdominal adhesions (they did it in my case). Do your research properly before getting your life ruined.

I am not sure if varicocele can be cured by tablets. Few medicines and lifestyle changes can prevent progression and reduce symptoms. But I don't think it can be cured.
In grade1, usually recommendation is conservative lifestyle changes and medicines. But again - the intent is that it should not progress. Not to reverse it completely

Not necessarily. It can mean that the lymph nodes are causing the varicocele due to pressure. Either ways. It requires further evaluation. Meet a specialist and get evaluated

Embolization only works if You have no Big external spermatic veins or gubernacular veins otherwise you will experiment relapse. Embolization does not reach out to the external spermatic veins neither gubernicular ones, so it really wont solve the problem in a big amount of patients. If you like to do sports or lift heavy weights then your blood will push on external and gubernicular veins wich leads to recurrence. Remember that embolization only treats internal plexus. Embolization recurrence is much higher than shown on the articles, same happens with the laparoscopic approach. The procedure that offers the higher success rate is the Microsurgery-with artery and lymphatics preservation including testicle deliver and use of micro vascular doppler device during surgery and removal of three groups of veins and letting only the deferential vein for blood return. This surgery must be done by urologist expert in Microsurgery so you can have the warranty that will be done properly and recurrence rates are less than 1% if done in this way.
Agreed with all points. Unfortunately, most microsurgeons do not perform testicular delivery or ligation of other 2 plexus. And only sub inguinal internal spermatic plexus ligation. And also, not everyone uses intra op doppler. To save arteries. Neither does everyone preserve lymphatics. So, we have seen less success rates compared to literature for microsurgery too. And higher complications rates including hydrocele and artery ligation inadvertently.

Most of urologist just use microscope without having previously training in Microsurgery in urology technic and don't use the microdoppler vascular device to avoid artery damage. Jajah rehta hai. Not dangerous. Just that left varicocele is common. Right or bilateral r rare. So, if it does occur, we rule out secondary causes. One of which can be grave or serious. But generally it's not.

No no. The coils never migrate once thrombosed and in place.

Embolization only works if You have no Big external spermatic veins or gubernacular veins otherwise you will experiment relapse. Embolization does not reach out to the external spermatic veins neither gubernicular ones, so it really wont solve the problem in a big amount of patients. If you like to do sports or lift heavy weights then your blood will push on external and gubernicular veins wich leads to recurrence. Remember that embolization only treats internal plexus. Embolization recurrence is much higher than shown on the articles, same happens with the laparoscopic approach. The procedure that offers the higher success rate is the Microsurgery-with artery and lymphatics preservation including testicle deliver and use of micro vascular doppler device during surgery and removal of three groups of veins and letting only the deferential vein for blood return. This surgery must be done by urologist expert in Microsurgery so you can have the warranty that will be done properly and recurrence rates are less than 1% if done in this way.
Agreed with all points. Unfortunately, most microsurgeons do not perform testicular delivery or ligation of other 2 plexus. And only sub inguinal internal spermatic plexus ligation. And also, not everyone uses intra op doppler. To save arteries. Neither does everyone preserve lymphatics.
So, we have seen less success rates compared to literature for microsurgery too. And higher complications rates including hydrocele and artery ligation inadvertently.
@@DrGauravGangwani totally agree, it goes hand in hand with the experience of the surgeon. Most of urologist just use microscope without having previously training in Microsurgery in urology technic and don't use the microdoppler vascular device to avoid artery damage. Jajah rehta hai. Not dangerous. Just that left varicocele is common. Right or bilateral r rare. So, if it does occur, we rule out secondary causes. One of which can be grave or serious. But generally it's not.
