Noticing how much hurt they caused person B by leaving them behind.
From the perspective of person B, the person they probably most cared about has left them. Now they have truly no one, they will forever feel at fault for not being able to save person A. They will hurt. And maybe even follow person A’s footsteps.

What I interpret from this song ( having not listened to the rest of the album yet) is that person A ( person who is singing) committed suicide and person B came desperately trying to help, but they were too late. And as person A tells, they died in their arms, at the last moment when they’ve fallen into the afterlife. They realized they screwed up and desperately try to get back, to get back to person B. Noticing how much hurt they caused person B by leaving them behind.
From the perspective of person B, the person they probably most cared about has left them. Now they have truly no one, they will forever feel at fault for not being able to save person A. They will hurt. And maybe even follow person A’s footsteps.
