He never died. He was in a coma for 5 years. He started singing at 12, and was in Route:L for 2 years, before Mitsuki was born. So he's between 24 and 26 at the start of the story, and between 29 and 31 at the end of the story, when Mitsuki is 16-17.

His former girlfriend was 25-26 at the final chapter (in volume 6 she's 22, a bit before the 3 year time-skip), and from the photo of the 2 together, and the story about the start of their relationship, Takuto seems to be about the same age. At most he would be a year younger than her. But yeah, he's a bit younger in the manga, as Mitsuki was apparently already born when he entered Route:L. He should be about 20 at the moment of his suicide attempt, 2 years before the start of the story. His shinigami form should be stuck at that age, while his body continue to age. In the last chapter, he would be 25-26, when Mitsuki is 16-17.
