Yeah bacteria can sometimes be found in sugarcanes if its rotten or has some unhealthy fertilizer fixed in it, but if the sugarcane is free from these then it's safe to eat
Sugar in itself is not unhealthy. Only bleached sugar which we eat regularly is unhealthy. Sugarcane juice has other healthy nutrients which help digest sugar.
