Well in this case it's a piccolo trumpet, for which 4 valves are reasonably common. But standard Bb Trumpets with 4 valves do exist, I think I remember trying a Stomvi with 4 valves a few years ago.
i think the fugue 7777 is a pun since the number 7 is pronounced "nana" and the main theme goes na nahhhhh
