    It looks like the vent needs to be opened and closed inside the fireplace. I would expect it to be operated outside of the fireplace, so my hands won't be burned when the fire is on.

    My 99 year old fireplace gets its combustion make up air from anywhere it can so it makes the rest of the house cold. When turning on the bathroom or kitchen fans they get their make up air from the fireplace since the chimney flu is now open. I'm going to email the company for advice because my brick fireplace sits inside the wooden home. In other words I have a combustible wall separating the back of my fireplace from the outside. Other than that this vent would help my fireplace add heat to our home.

      The combustible wall should not be a problem, you can use either galvanized or stainless steel pipe (rigid or flexible) to connect the vent and route to an exterior wall of your house. Just make sure to maintain at least a 1" clearance to any combustibles around the pipe.     My combustion air vent gets very very cold - ice even forms around the edge when it's ~20 degrees F and the vent is closed. Is this expected? Is there a way to seal it? Even with the flue closed the room gets very cold around the fireplace.

    what if there is minus 30 deg celsius outside? do i need any air preheater inside the pipe which is heated air before pipe run into house?

      You do not need anything to preheat the air. As long as the vent is open while the fire is burning it will draw in as much air as it needs regardless of outside air temp. Just make sure to close the vent when the fireplace is not in use to prevent any drafts.
