By the looks of it, I think it reverses the rotation of the planets, but the songs and timing of tiles still play normally. As a result, the planets now spin faster or slower to compensate for the proper timing (especially in angled turns), making it just a lot harder to read lol.
you see how those right angles speed up and slow down? they don't normally do that. usually when you take an inside angle, you have to click faster, while outside angles give you longer. Appril Fools mode inverts that. basically it just flips the track layout . while keepingthe timing the same as normal.
I was wondering if this secretly might have been an interesting development bug put back into the game for April fool's. Sounds like a mistake I would make!

Imagine you know that in a certain number of frames the circle should reach the next tile, so you code it to rotate the correct amount each frame to reach that target. If you flipped a sign around, the calculation could send the circle the wrong way round, while still reaching the right position. Effectively changing the speed.
Honestly? Night Wander looks so much more satisfying like this. The planets are doing more acrobatic stuff to get rid of the obstacles, and it's great!

I didn't know it's even a thing, I wanna play Third sun with it now, looks fun. (with exception of buggy mess of triple balls). I wish it was more known mechanic you can turn on and off for fun, it looks like a fun skill to learn - reading tiles \ patterns other way around.

I can't imagine playing ADOFAI like this, it would completely ruin my muscle memory. This is coming from a player who has 100% completed the game with all achievements.
system preferences > accessibility > keyboard > viewer you can create a new keyboard using the panel editor and edit it to only show the keys you use, then enable the keyboard and click the three dots in the top right corner > custom panels > the keyboard you made hope that helps 👍

yeah since the mode does not really change the song timings, its just visual
actually the song timings are changed, because the orbit is now (usually) faster timings get way more stricter
