If you want to know of AFO's intentions in this episode and why she did what she did then you must understand the concept of the Quirk Singularity. It is stated by Dr. Garaki that as the generations go on quirks become more complex and more powerful but as newer and newer generations come into play the quirks become too much. Their too complex, too powerful for the human body to handle that no one could control their quirk anymore.
And in this AU; AFO believed that to be true as well, but she came to this conclusion on her own as she didnt meet or learn of Dr. Garaki yet as the events of this episode take course many years before the events of the canon universe.
So AFO wanted to create the Nomu's way earlier than in the canon universe and this was way before she met Dr. Garaki and thus she held her own experiments
But you may be asking yourself, "Why would AFO be creating the Nomu's in this universe?" And thats where you need to realize that AFO's intentions in this AU is still the same as in canon. She wants everyone to rely on her so she could have control over the world.
I know that isnt really what it is if you know about it but just trust me on this, since AFO came to the concept of the Quirk Singularity on her own which seems to be true to her as quirks are becoming more powerful over the years she can take away everyone's quirks as she can store up to as many quirks as she wants into her body which'll make everyone rely on her for that in turn will make her able to rule the world since who'll stop her when no one can control their quirks or when everyone is quirkless? No one.
She already made a big impact when quirks first appeared and when people's quirks do begin getting out of hand and become too destructive they'll turn to the one person that can get rid of their quirks - AFO.
And AFO wants to capitalise on that and make her own army with these powerful quirks when the time ever came to enact her plan which is why she wanted to create the Nomu's way earlier than normal so she could have vessels to inherit these quirks, serve under her and rule the world as their eternal ruler.
But she still has to deal with All Might and OFA.
Now, how does she do this on her own when she had help from Dr. Garaki in the canon universe? With the use of these 'Cell Expanding' drugs mentioned early on in the episode.
It is known that the normal human body can only handle one quirk per person and if they were to have a second quirk there would be heavy drawbacks but AFO knows that but to create this powerful army of hers she needs them to have multiple quirks.
These experiments were all tested on living people, these people couldnt handle more than four quirks before their bodies imploded and they died from the massive strain and thats where she believes the 'Cell Expanding' drugs will help, more specifically the ones that increase the Quirk Cells in one's body.
When used the drug will create more Quirk Cells in one's body and makes their quirks more powerful but this drug takes some time to kick in, one to two hours time. In this gap of time the new Quirk Cells made by the drug will be a blank slate, normally in this time period the pre existing Quirk Cells would mold the new Quirk Cells to become like them but AFO wanted to use this gap of time where the new Quirk Cells are blank from the drug and make it so the blank Quirk Cells inherit the powers of another quirk when she transfers over another quirk in turn causing no harm or strain to the person, except of course, their brain. That in turn creates the Nomu's we all know (According to AFO).
And yes these blank Quirk Cells can get infected by diseases or any illnesses which causes the drug to not work in turn rendering the Quirk Cells formed by the drug completely useless.
