    Best video I've seen on how a cochlear implant works. The booklet I received before my surgery downplayed the pain and seriousness of the surgery because it is just an out-patient procedure. It is still a surgery. You can see the incision - your ear is peeled back to have room to do the implant procedure. I had my surgery 2 months ago. I was activated one month later and it was a miracle. I heard speech immediately (okay, Minnie Mouse, but every word was clear). But, I am still having numbness and pain in my incision area. My skull is tender. I'm sure I have more healing to do. My ear is swollen still (but improved), and as the processor sits on that ear, I am sore by the end of the day.
    I know that everyone reacts differently to a surgery and I'm not a spring chicken (69), and I would do it again in a heartbeat. Yet, I would say, don't make it sound like the surgery is a snap. Its not.

    I enjoyed and have learned so many important hearing loss and hearing aids aid information from your RUclips and podcast! I recently suffered Sudden Neurological Hearing Loss and loss hearing in both ears. You RUclips are keeping informed of the importance of hearing treatments and hearing aids. You are keeping alive and hope for the future. I am in a process of CI evaluation for the left dead ear and feeling more confidence because of your information in RUclips.     Going through the process right now, had the hearing test done , was giving the 3 manufacturers to pick from and think about and last week 2nd meeting to pick my choice “Nucleus 7 “and finally next week get to meet the MD.Thanks for all your videos.

      @@jaimalhotra2071 In Germany, the device and the operations and accessories are only paid for by health insurance if there is a hearing loss or deafness, so I don't know how much it costs in other countries.

      I've had my Nucleus 6 for 3 years now and I suffered from severe tinnitus which made hearing and listening so hard for me. The tinnitus doesn't go away but what's great about the cochlear implant is that the tinnitus TEMPORARILY GOES AWAY while you're WEARING your implant (with it turned ON of course) Once it's turned off or taken off your ear, the tinnitus comes back a little bit but it's not as loud as it used to be before the implant. It doesn't bother you because you will be used to hearing so many more sounds that your hearing aid can't pick up that you will forget about the tinnitus. You're giving your brain a break when you turn it off. Tinnitus used to be a real big issue for me when I used to wear hearing aids (before I got my cochlear implant) but after my implant, it's barely noticeable and you can only hear it if you think about it lol. I totally forgot about tinnitus until I saw your comment asking about it. It doesn't bother me anymore like it used to. Good luck to you.

    Thank you for this update. Are there 24 true electrodes and which of the approx. 99 frequency chambers do they target and how do they prevent electrical spread in the ion rich cochlea?
    I like that you mention at 06:53 "...preserve your cochlea just in case there are some other restorative treatment options down the road."
    If I've not been clear or made something unclear, please feel free to correct me And explain in response to benefit everyone's understanding.

    I don't know what to expect for activation day a little scary I pray 🙏 that it will be ok I just got my implant about three weeks ago and it's almost a month already so time has been going and I'm glad

    I have the N7 cochlear but the battery drains so fast! with my n5 before my upgrade it lasted approx 1 1/2 days but with my n7 it lasts about 9-12 hours. I turn off bluetooth on my iphone and have the scan program.

    Cliff, I really appreciate your video review of the Nucleus 7. I just had my first implant four weeks ago and was activated about a week ago. It's a brand new hearing experience, and although I am hearing only beeps (like a fax machine) at this point, my audiologist told me I was already hearing sounds in the 8,000 Hz range (my max before Ci surgery was about 1,000 Hz; I have profound loss in both ears). You mentioned things you would like to see Cochlear offer in their Ci's- I'd love to see them develop their "plastics" (as one audiologist calls the case design). I wish the Nucleus 7 case had a more round exterior, rather than a squared exterior with mildly rounded edges. I think it would make for a more comfortable and snug fit. The Cochlear company seems incredibly helpful to its customers; such is one reason my audiologist recommended the Cochlear brand.

      Agreed! I am also profoundly deaf (genetics) so I had my first cochlear implant when I was 3, and then another operation when I was 12 or 13 to get my other ear (that doesn't have an implant inside) implanted. Which means i have two cochlear implants. I'm 22 now, and I can tell you - it's awesome! It gets better overtime because of experiences, also with the way technhology is advancing, I have no doubt we will get even better accessible things for the upcoming cochlear implants :D

    Dr. Cliff I love your videos and above all your practical, guiding, revealing and optimistic comments, it would be very interesting if you talked not only about Kanso, but also about Rondo 1 and 2 and about the direction that hearing technology will take so as not to be taken out of the market with the advent of increasingly successful cell and drug therapies, for example the efforts of Frequency therapeutics and neuralink.

    My effort to benefit from Cochlear implant has been frustrating and very time consuming. Use of Cochlear for telephone calls is one outstanding problem. My audiologist advises that this limitation persists for some patients of Cochlear. Currently I am using AirPods Pro to overcome this limitation. I am confident that your research and knowledge of Cochlear had reviewed this problem and found solutions to use Cochlear directly for telephone conversation.
    I would like your recommendation to use Cochlear directly for telephone conversation

    Thank you very much for the explanatory video. Let me begin by saying that I'm brand new at all this.. I was operated on two weeks ago (labyrinthectory + implant), and my implant was activated two days ago. Baby steps. I've been wearing the Nucleus processor that clips over the ear. I would like to ask whether it's normal / usual for the clip to cause pain where the clip ends (i.e., toward my face) after wearing it 7 hours one day and 8 hours the next day. I feel tender and sore. Today, to get a little relief, I switched to the Kanso processor that I got at the same time. Not having to wear an earpiece gives relief, even though I'm somewhat concerned that the Kanso will fall off. I also have tenderness in the area where the magnet is implanted (the internal part). I'm trying to do what my doctor said ("wear it as long as you can stand it"), but with ongoing pain and tenderness, wow, it's a little tough. Any suggestions? Many thanks.

    I got a cochlear implant approx. 3 years ago. I have the Nucleus 7 Sound Processor. The problem that I'm having is the sounds I'm hearing with the processor are very garbled. My audiologist has tried various settings with no real improvement. They even checked to make sure the implant wasn't defective. (it wasn't). Is there anything else I can do to improve the sound reception of my Nucleus 7 Processor?

    Well done once again. Apparently Oticon have released a new baha, the Ponto 4. It would be good to have a review of that out of curiosity. I have the baha five, though it would be interesting just to hear a review of the Otocon Ponto 4. Good job once again.

      I had my implant several months ago. Because I wear glasses I was very interested in receiving the off the ear Kanso processor. Unfortunately I found it a bit too bulky and would inadvertently knock it it off my head when I’d be sitting in a living room chair and turn my head sideways or when I’d get in the car and turn to put my seatbelt on. After my first two weeks I decided to switch to the over the ear Nucleus 7 and Cochlear made the switch for me. My hope is that with tech advances, a slimmer off the ear version will be available in a few years.

    For two after my processor has been turned on, all I get is a noise like a whistle. I was told that it will take at least six months for this processor, to work.

    I have the Nucleus 22 and the Nucleus 7. I was told that the Nucleus 7 has options of adjusting the Treble and Bass volumes, but I was told the Nucleus 22 is not capable of receiving those treble and bass volume adjustments.
    Is that true? Please tell me soon.

    Hello Dr Cliff, This Peter P. A senior at 70 and live in Canada. My left eye is entirely deafen, I am scheduled for an assessment to do cochlear implants at a local hospital. They want to make sure I am qualified. It may not be for medical reason but the draining of public funding as this free to Canadian. It is a long wait. May I know where you are in the states and what is the ballpark cost of the implant at your hospital. And with nucleus 7. Many thanks

    i am going deaf due to acoustic trauma, i am 23 years old. due to the acoustic trauma, i have chronic eustachian tube dysfunction which as of now, there is no cure for, only temporary terrible half solutions, that only sometimes work, and have to be re done yearly. i heard implants cause alot of ear pressure. i already have so much pressure in my ears, i fear one day one of my ear drums will rupture. will i never be a candidate.

    Wait, wait, wait, Dr. Cliff! You said that the implant connects to your T-Coil. Nope...it connects to your coil which connects to your processor. But easier to say it connects to your processor. It is hard to talk about a CI when you don't use one yourself. But I admire you for jumping in there!

    My audiologists at the VA told me to start thinking about cochleae implants, but she said the sound wasn’t normal and it may take as much as a year to learn to hear again. What can a person expect to hear after the surgery.

      Cochlear Implants are amazing. Everyone reaponfs differently, but most people experience significant success pretty quickly.

      It depends on how quick the brain responds to the new sounds it hears. If you're into music and FLStudio, you'll be quicker at identifying different sounds you're exposed to. After my first switch on, everyone sounded like children and like robots with high voices. My audiologist told me most people can't understand speech straight away after switch on. She said most can hear sound but couldn't understand the speech they were hearing. She also mentioned to me that everything you see on RUclips of people hearing straight away after switch on was fake. But I proved them wrong that day as I became their first implantee to be able to hear and understand everything they were saying straight after switch on and boy was I proud lol. The cochlear implant is beyond what I call amazing. I would never ever go back to wearing a hearing aid after this. That would be a massive downgrade.

    I am a patio musician that is deaf in one ear. I have Ménière’s disease in the good ear. The Ménière’s is non vertigo like others but the swooshing tinnitus comes and goes and when I have it, I cannot hear my guitar well enough to sing. Do you know if this may help musicians?

    Hi! I have a Baha but my doctor said I would be a great candidate for CI but that she wouldn’t recommend it because of the learning curve. Do you think I could make this switch without surgery? Or is it even advisable at this point?

    I just subscribed I had my surgery yesterday for my CA CI. I am getting the N7 and the other one that sticks directly to the magnet.
    I have a resound in my other ear and depend on a phone clip.
    I hope I made the right choice in going with CA.
    I was born android. It would be hard to switch.

    Mi scusi se la disturbo dottore,mia figlia di 10 ha iniziato gli esami per la compatibilità con impianto cocleare per una sordità medio/grave. In Italia del Nucleus 8 non si sa nulla,lei saprebbe dirmi se è in programma un prossima uscita imminente? In Italia si può sostituire ogni 8 anni e non vorrei che gli dessero N7 comunque efficiente ma datato con la prossima uscita dell'8. Grazie mille se avrà tempo e modo di rispondere e grazie per i suoi fantastici video

    Great video, appreciate this - currently considering an implant myself! When you talk about preserving your cochlear (ie non implanted side) incase other restorative treatments become available, I'm a confused as I thought CI surgical techniques have improved in such a way that they can preserve residual hearing. Could you provide a little more clarity on this and perhaps what restorative techniques might be down the line?

      I think, whilst improved, there’s still something like a 10-15db loss of residual hearing in the implanted ear.

    Hi Dr Cliff
    I am ready for surgery on my SECOND ear. Due to mitochrondial diseases, I've been rendered biologically deaf.. Right now I get by with singular N6. Ready for surgery, and installation of N7. In the downtime before surgery, I had my audiologist send me the N7 as an upgrade over the N6.. The N7 which arrived came with batteries and charger; of course I got everything charged up. Then, with the N7 out, zero. results. I found out that the N7 needs to first be PROGRAMMED by someone with similar credentials to yourself (my audiologist). My questions are these:
    [1] why? There are a few options to review, and in every case I would answer "yes I want this." If the new unit needs 'programming', why wouldn't the company send everything to the audiologist involved? I hope that you as a professional would prefer sending new units to audiologists.
    [2] Why does the extensive documentation which came with the N7 (more than 6 books) have NO MENTION of the need for 'programming' ?? A new owner/user would certainly need to be reminded of this!
    Thx for factual review. I have minor disagreement with your facts: You state that Apple products get instant streaming, but Android users are denied this feature. Every company pitch has been to the opposite. Please double-check with Cochlear Inc, and then check with an actual Android owner who is limited to implants.

    My son just got the nucleus 7 in one ear.
    The other ear he can hear very well even without a hearing aid.
    My question is why do some cochlear implants have a piece that goes into the ear like a hearing aid and some like the nucleus 7 doesn't ?

      I got my N7 Sept. of last year, it can stream to either phones but you will need to get a phone adapter to use with a android phone. The N7 streams directly to the iPhone.

      Not a whole lot of differences when programming the Hearing aid in a Bimodal situation. The main focus would be to link the two devices together if possible to match program settings and the ability to stream sound binaurally.

      Is there a way for a spouse, teacher or friend to temporarily interrupt the streaming, kind of like a walkie talkie? My husband is both deaf and blind and streams books, movies and music all the time. There is no way for me to know if he is streaming, and he can't see me getting his attention.
      I would love for there to be a way that I could have a device that I could tap a button, it would temporarily pause what he was listening to, say something to him then release button and it would go back to what he was listening to. Sure seems like it could be done.

      My son has severe hearing loss in one ear and profound in the other. We've been recommended to get implants in both ears.

    My wife lost her hearing in one ear many years ago, I’m worried about her loosing the other ear. Do you think this would be able to help her in the ear she lost her hearing?

      Generally if the hearing aid is not helping, CI is the next step but she needs to see an audiologist MD and an Otolaryngologist. There's a group called cochlear implant experiences, join it on FB. No sales, no medical advices. If you post it there, you get an answers based on users 's experience. Good luck to you both.

    Words of CAUTION:
    I chose the Kanso processor to eliminate the behind the ear pressure / pain, I wear glasses. I received the most resent Kanso processor that is MRI compatible. The strongest magnet strength available is a #4. The #4 magnet is barely strong enough to hold the processor onto my head. The slightest contact from anything detaches the processor. This is VERY annoying. I am very disappointed in Cochlear Americas for not properly informing me.
    I am now being forced to settle for the N7 processor and deal with top of the ear pain. I was informed by the Audi that Cochlear has no plans to increase magnet strength for the Kanso. Had I know this prior to surgery, I would have chosen an AB or a Med-El system.

      Well the device is very small, maybe the processor can't hold a stronger magnet yet. Unfortunately there's nothing they can do about the ear pressure pain. They're not doctors. Their goal is to help us "hear" better not "feel" better... Well they want us to feel better after we hear better. It is not their fault you're in pain. Maybe check in with your doctor for pain killers? Or ask for frameless or weightless glasses? They're working hard on making the next generation CI a lot smaller and lighter and it's very amazing how far they've come. I have the Nucleus 6 and I'm in the process of upgrading to Nucleus 7. Nucleus 6 is light for me and the magnet strength is #6 which is the strongest. The N7 is apparently lighter than the N6 yet the N6 is already super light. I'm sure you will have no issues with the weight or magnet strength since the N7 is super light. Sorry to sound harsh. But best wishes to you and may God take away your pain.

      I had the same issue when I got my kanso 2 so I decided to get a sweat bad to help hold my kanso 2 in place and it works great I like the kanso 2 because I wear glasses and you know we have to wear masks also and that's a lot for one ear to hold and it gets sore with the elastic from the mask 😷 so get a sweat band and you can see the difference you I got one from Dick's sporting goods and of course you can get one from cochlear also ok so it's something you can think about doing

    Love your channel! Just a quick question, I saw Cochlear website that they also have an acoustic implant called Carina, can I ask what is the difference between this Carina line and the Nucleus line?

      It is basically a hearing aid that is implanted. It has similar outcomes to hearing aids it just doesn't have any external components.

    Hey my son has 2 nucleus 7 cochlear implants and have had them for 2 years now his learning and speech has improved alot since having them.
    Quick question with the nucleus 7 one implant light is green and solid but my son said it wont go tryd alot of things but cant get it to go do you have any ideas

    I just got second surgery of Cochlea Implant.
    And I’m sooo excited about my new cochlea implant! This is like from the future!
    I have question, like you said that the sounds from TV can go in the Cochlea Implant, does that mean ,for exemple, when I play PS4, and I want sound but my family doesn’t want any noise or sounds, does that mean that I will able the only one can hear the sound from PS4, right?
    like Bluetooth Headphones ?

      @@idraconixi i have a nucleus 5 in my left ear which just got upgraded to nucleus 7 and i got surgery on my right ear dec 4 got the newer nucleus 7 device put in and it get activated in January so now ill have 2 nucleus 7

    I have single-sided deafness. If I got a cochlear implant for my good ear, would that totally kill the hearing in that ear. I have concerns about safety when I am not wearing it. Also what about an implant in my bad ear which has been bad for decades. Thanks for the video.

      Douglas Smith you need to speak to a cochlear surgeon to answer those questions but it’s possible you’ll lose rest of your hearing yes.

      Why would you want to put it in your good ear and not your deaf ear?
      I’m born SSD, and now I’m different kind of SSD.
      I have bilateral cochlear implants and they work well, my left ear may never be as strong as my right ear but together they work great, not bad for getting both of my implants at the same time.
      My recommendation is to talk to your doctor about it.
      I will say that the good ear for me progressed a heck of a lot faster than my deaf ear did. My deaf ear is progressing really slowly and only now after 7 years that I’m finally able to use the phone on with the deaf side as well as carry on a conversation.

      Yes. It wil help him very much. You have to go for implant if its severe to profound hearing loss. But Cochlear implant N7 is a bit costly.

      Dustin Sanders you can’t compare. Hearing aids mean you still have residual hearing so it will never sound the same as natural hearing ad it’s electronic and its ridiculous when sine audiologists assume you hear better. Ok speech may be heard better but it’s not like all sounds are as a whole and it’s robotic but if you can no longer hear speech then yes it’s better than not hearing speech.

    N7 was FDA approved for Android in June 2019 not long after this video was aired so you can now use it on both iPhone and Android!
    I like the sound of the CROs option! One has to wonder why it’s not already an option other than $$$? Unfortunately I was not told to get resound when I purchased my new only a few months before surgery and the cochlear department at the hospital were aware of this too so I cannot stream. I’m considering changing to resound though when I can afford to.
    Ps my surgery was over night and hurt like hell lol. Painless...not lol

      Yes, you can use ReSound accessories, but you can get them as part of your implant package as well to save even more money.

      In my experience, the disposable batteries lasted ~7-9 hours, and the rechargeable batteries start with ~10-12 hours, though as they age it dwindled down to ~5-7 hours

      You probably can't. I tried to wear a motorbike helmet over my implant but it kept slipping off and wouldn't stay on. That happened to me only because the helmet was tight (even though it was huge! Not sure if that's because I have a big head or what lol) maybe you can if you have the correct size helmet that fits your head?

      I have a Phonac hearing aid, it not electronically connected to the CI. You should be able to use any HA.
