I understand your comment, but If you don't mind, maybe there are two tiny points in this narrative you may not have considered? 1. Maybe the "kidney" from his wife is not a compatible tissue, and his body will reject it. That mismatch is very serious, but maybe the donor's kidney is a viable match for his body. 2. The ex-girlfriend character is apparently making her decision to donate her own out of true love for the recipient. A love deeper than her disappointment over unrequited desire to be together with him -- a love that is unselfishly wishing only the very best for the recipient: hopefully good health and a happy life, even now when she is not a part of his life. Maybe, sometimes, that kind of love may exist, if so, it will probably not appear as attractively romantic as a movie theme played by beautiful, famous movie stars. Maybe? Sometimes? And moreover there are some who went to moon so not to be surprised when they can know about someones identity.
