You know, I never considered this but that is actually right. Doctors should really be wearing something to prevent infection with HIV, I don't know if that's a inaccuracy with the show or a oversight in the hospital security
In real life they most likely would have been masked, likely with face shields instead of fabric face masks, just in case. But the problem with showing that on TV is that actors are expensive and you want to see their face and expressions as much as possible, as that’s what you’re paying for. That’s why medical dramas rarely have people using masks, even when they’re supposed to. But some people are also careless, and Cameron has been shown to be unreasonably trusting, and HIV does not transmit through the air, so she may have been overconfident.
As a nurse, when we take care of patients with HIV, we have to wear gloves when it comes to blood work and things like that. But also, before the pandemic I don't believe people wore masks. It's rare to pass HIV through saliva unless they have open sores in their mouth. It is contagious through blood, semen, and breast milk. But a mask and face shield probably would have helped her in this case because of the blood he coughed up.

When he started coughing she probably should have gotten a mask, not for HIV but the thousand other things it could be. When he said he saw blood she definitely should have gotten a mask. He fired 13 because she got caught by Cuddy and he was saving her from a mandatory drug test that would’ve killed her career. If her and Cameron’s circumstances were swapped, I think House would’ve done the same thing in both cases.
