    Great video very informative for someone such as myself at 64 that is trying to push doctors today. You have to just buy the numbers to get off their ass and do something and my number have continued to drop diabetes type two pulmonary problems severe sepsis eight years ago, never been right since primary hypertension, CHF with ejection fraction below 50 and that was two years ago my doctors in Boston next week going to push to have all the test redone and then I have a cardiologist in the area I live that’s going to repeat all the same test and see what he comes up with in the meantime, I’m going on colonoscopy endoscopy within the last 16 months next one coming up within 90 days diverticulosis diverticulitis, gastritis, and now all these lesions constant headaches which I’ve always suffered from migraines and have taken medication for, but this is different type of pain for me. This was very informative and I appreciate it, sometimes you feel like you out in the ocean in a small boat with raging waters no food no water no life jacket and hoping the Coast Guard comes along is the way you feel? again thank you

      Severe Anemia causes volume overload ,tachycardia and high stroke volume so these factors lead to cardiac failure

    This is wrong, it’s hypertensive heart failure, which is different from what we’re calling HFpEF.
    The echocardiogram is different, and so is the pathophysiological process.
    The treatment is different for hypertensive heart failure - which is antihypertensives, and HFpEF which is metabolic control - using gliflozins.
    HFpEF is specific to echocardiogram that is NOT hypertrophic, and blood pressure is normal or LOW, with traditional heart failure signs and symptoms. The cardiac muscles instead contracts poorly/hypodynamicaly/slowly so an ejection pulse is tapered out and weak despite the overall ejection fraction at end of systole being of a normal fraction.
    If there is hypertension, by definition the so called cardiac failure is not due to insufficiency, but excess.
    I recommend checking out carolyne s.p. Lam cardiology grand round on RUclips for an updated understanding of what HFpEF is, which is different from hypertensive heart failure.

      Also, we don’t only classify heart failure into hfpef and hfref - that is medical student exam level knowledge which is about 10-20 years behind what current medicine is. You learn this to pass exams but it is wrong.
      You did not also indicate high output cardiac failure, where despite a relatively normal heart function, the lack of systemic vascular resistance results in an insufficiency of cardiac output to meet an abnormally high output demand I.e in septic shock, beri beri, hyper thyroidism etc.
      Also, diastolic failure is also due to excessive ventricular rate like fast AF or other svt(of which there are a bunch)
      There are various causes of heart failure which are not necessarily due to the heart itself but rather outside of it, and then those due to the heart. They can all resemble HFpEF if you use the blanket terminology, but their pathophysiological processes and treatments are so different that it is not right to lump them together merely for heuristic short cutting.
