What is the difference between Psychosis and Psychopathy?

    Psychosis vs psychopathy, the words sound similar, are they the same thing? I think this is a common question and people have a misconception that the two concepts are similar. But they are completely unrelated.
    Psychosis is a mental state where you sense of reality is altered. When you are in this state you have trouble knowing what's real and what's not real. There are different ways you can manifest psychosis. Psychopathy on the other hand is a set of personality traits whereby people disregard the rights of others and their behavior conflicts with social norms. In this video I discuss two models of psychopathy to help you understand how it is different from psychosis.

    having experienced psychosis I can confirm it is completely different to psychopathy :) with a psychopath having a lack of empathy, a person with psychosis usually has a surplus of empathy

      So well said.. I’ve had episodes of psychosis where I lost my mind over someone mistreating my disabled spouse in a medical situation .. I’ve also had episodes where someone treated me with complete lack of respect and I’ve lost my cool .. I’ve lost it with people demanding money on the phone (health insurance company /car insurance etc..) when we were told it was covered .. I go into this tunnel and all I see is anger , and their attempt to ruin my life and leave me homeless and dying on the street . Must be because when I was a kid , I was beaten , my sister was harassed and ended up not with us anymore ..then I had to fight my dad and he tried to strangle the life out of me..but I got him down ..he never touched me again , but he passed his form of rage onto me as a coping mech. I’ve been trying to unlearn it for 45 years .. it feels agonizing after I come down .. to realize the hurt I’ve caused with words .. people I’ve never met on the phone scrutinized , ridiculed , so vociferously. Sometimes I cry because it feels like I can’t stop . It’s like I fill up with cold water as soon as I hear someone’s tone take on that hostile nature my dad and his Christian church preachers had. Any kind of persecution, hostility , shaming or condemnation and I go into defense mode and battlestations

    Dr. Marks thank you for your videos and information. You're my favorite mental illness channel. When I was 21 I had an episode of major depression with psychotic features (I'm 49 years old now). I enjoy how accurate your examples are and you say them in a way that that people can understand. Again, thank you

    I really appreciate this video. After housing my schizophrenic FIL for the past 6 months and dealing with all his episodes of psychosis was really rough and some times scary. We had no idea how to help him, since he was under the belief he wasn't "crazy", and he didn't like how his meds made him feel, thinking he was better and didn't need them (only cause the drugs were working) He just moved out last weekend. We feel bad but also so relieved. It's hard to help someone who wont/can't help themselves.

      You're right. It almost like you have to watch them crash and burn and end up getting urgent or emergent care before you can start to make some changes.

    I already knew most of this, but it’s great to see such a thorough explanation. I think it would be good to also do a video on psychopathy vs sociopathy, as I’m not sure I fully understand that… maybe also including narcissistic personality disorder?

    If you have met a psychopath, you have my sympathy. Those folks are dangerous. Never, ever trust one. Run, do not walk, from working for or being in relationship with a psychopath. Psychotics, on the other hand, aren't always dangerous. Just sometime. I treated a nice young man who killed the family cat when psychotic, and then did the same to a sports car. I never turned my back on him. I did the same with the female prisoners when I taught infant care classes. Then I went back to pediatrics. Less work, more fun.

    I'm a retired forensic captain and specialized in training LEO's, nurses, MD's, social workers, etc. in working with the criminally insane and I thoroughly enjoy your channel.
    Could you possibly make a video covering Schadenfreude. Taking pleasure in another's misfortune. Two people in my life that I love and care about very much show very strong signs of this trait. To the point of pathological lying, kleptomania, and out right sabotage to see the inconvenience and failure of others. I'd love to hear your view and ways of dealing with these individuals.

      It's nice to see neighbors supporting one another, you don't see that very often. It's very kind of you. Psychosis is very scary for all parties.
      One tip, make sure your safety comes first. When in psychosis people can get irritable, aggressive, want to hurt themselves or others, they might even perceive you as a threat. Their thought process is totally skewed (I know I've been in a psychotic episode before).
      Listen to your gut and if need be, (because you have to sometimes) call 911. Your safety comes first and foremost.

    Great video, as usual. I've just had a pretty severe period of psychotic depression and even though I'm stable (I have bipolar type 1), recovery hasn't been easy. Could you make a video talking about the recovery process and length from different situations (depression, mania, psychotic depression, mixed episodes, etc...)? Please? I'm sure more people would appreciate it, too.

      Hi Reinaldo. Thanks for the question. The recovery process is different for each person not only depending on what they are recovering from, but also what's going on in their lives, their past recovery, etc.. I wouldn't be able to give general predictions or expectations.

      Some of the recovery trajectory depends on factors like anosognosia (insight’s presence/absence etc), social support, housing and financial stability, access to medications and how long the illness has gone on, esp if untreated/undertreated. That is why there is such emphasis on the importance of ‘First episode psychosis’ clinics for early intervention.

    My good neighbor and friend has psychosis I believe. We hang out all the time we go so many places and he just flips the script every effin time. And it upsets me so much because he's so normal but in public I guess because that's where I am at with him most I see him flip. It makes me need counseling because I get to wired up and frustrated but I love him because he's a cool friend to have to we are both stuck ...🤯

    Last year I also had this belief that I would harm someone, those nights of anxiety and hallucinations were horrible, I was sweating, it was strange, I didn't know what was happening, I think they seemed like hallucinations, I don't know, those thoughts and sensations went away and I was fine again. temporary although it lasted several months, but recently it has returned to me, and is weird, I think it's because I started to overthink how I acted sometimes and my head told me that there were things that I didn't care about, I walked away from two friends because my head said that I didn't love them and it's confusing not knowing how to distinguish reality from that, Right now I feel bad and strange, I want to act well but I'm really impulsive and I get upset about things, I hope it's because of my teenage stage and that's it :(

    Hi Dr. Tracey Marks! I love this video and your content!! Question: in psychiatry, where is the line where a medical delusion starts? I ask this because when I had psychotic symptoms in the past, I did have some delusional thoughts. However, I always knew deep down that these delusions are not LOGICALLY possible. I still acted on them and did things to prevent what I feared, but I did it more for prevention in case it was real, not because I fully believed it. Would this be described as a delusion or just paranoia? Can someone have a "medical" delusion while logically recognizing that the belief is inherently unlikely or false? Thanks again for this video!

    Very good & clear explanation.
    I have an acquaintance who insists on using the word psychotic when he means psychopathic.
    Several times I have explained the difference but he takes no notice!
    Perhaps he will pay attention to this?

    i lost a friend today over this argument. She kept on saying that psychopaths are sociopaths with psychosis. Which is just wrong. She kept on painting me as a bigot for correcting her and got our mutual friend to side with her. So i lost him too. I can’t believe this all happened and i’m still right. It’s annoying.

    Hello, Dr. Marks. I wonder if you can make a video about euthymia(normal mood) i wonder if euthymia existed in MDD or PDD? I know euthymia existed in bipolar too, can you describe euthymia in bipolar too?

      Ok let me see if I can make a whole video about that. That's more like video-ette 😃 The short answer is euthymia is a state of normal mood. It's the state you are in when you are in between episodes, assuming all of your symptoms go away. Not everyone's does. Ok, I think I may be able to have a few things to say about this. Let me see.

      @@DrTraceyMarks what is "normal" in someone with a mental illness. My normal before I got sick and my normal since then are COMPLETELY different. I find that people (well, I know I do) make due with what the mental illness didn't take and out of that I have made a "new normal".

      @@DrTraceyMarks I know that euthymia is normal mood. But sometimes, I got confused with "euthymia" vs (hypo) mania. Both of them feeling good within myself. But, i wonder if I'm actually mania or euthymia, because my suicidal thoughts went "poofed" suddenly.
      Does euthymia means there will be another mania/depressive episode upcoming?

    I’m that person who tries to talk about something and end up on tangents .. I don’t like speaking to people for extended periods of time , unless I’m coaching them and then I focus entirely on their need and their desired and stated goals . If I’m left to my own devices , I get easily sidetracked and often end up making several repeat loops around my insecurities and failings . ***trigger warning ***I was beaten as a child by an evangelical father up to the age of 14 , when my sister was driven to unalive herself . I am medicated and under the constant care of a psychiatrist. I’ve been married /partnered for 23 years and live in the same house for 12 years. Psychosis happens for me whenever my disabled partner is treated poorly by rude clerks at hospitals or clinics . I’m his caregiver 24/7 and I find it very difficult to care for myself . I tend to focus on the pain and injury of others (like my mother )

I speak and the image bis in my head, I know what I want to say but another word comes out. i didn't realize I did this until my daughters told me. I didn't understand that people could not comprehend my sentences until after it was pointed out to me.

      Hmmm...let me see what I can do with that. The short answer is the trauma practitioner is also a therapist who specializes in trauma based therapies like EMDR, CPT, and PET (exposure therapy)

    Great breakdown of these two terms. Psychosis and psychopathy could not be more different. Psychopaths will exploit and manipulate people with psychosis all day, every day and sleep just fine at night. Psychopaths are almost entirely morally bankrupt and ruthlessly greed driven.
    I think it might be the term "psycho" that has people confused with these two terms, not psych (which most people use as an abbreviation for an academic class). I hear people misuse the term "psycho" quite often, especially online.
    Some psychopaths are sadistic, but I think that is more people who are borderlines and malignant narcissists, as well as those with sadistic personality disorder.

      I had to go through rehab, for alcohol abuse, but I did learn that Psychosis does go away...maybe not 100% but it is manageable IMHO, Therapist actually helped me (but this was alcohol)

      It can if it's a one time experience AND you're not vulnerable to psychosis on your own. Some people's psychiatric issues like OCD, anxiety and delusional disorder can be activated by drug intoxication.

    I always struggled with myself because of my huge lack of empathy. There was a point where I even tried to kill my brother, he’s alive and well. I’m not fairly strong and all I had was a pillow. Yet afterwards I just felt pissed off, and even now I still have thoughts and wishes to put harm on literally anybody. I even attempted to cause a car accident.
    However, my therapist has had thorough talks with me. I honestly just wanna live a normal life but all of these violent fantasies never go away. So for now, we’re just trying to work on my terrible addictions

      That sounds more like a control issue than psychosis, maybe you have it too but idk. About the lack of empathy, hldo you have depression or any sort of problem?

      You seem normal because you wanna live a normal life without harming anyone and you are conscious about how terrible those fantasies are. You probably have intrusive thoughts too and OCD.

      @@imsh00k I have a whole history, since I’ve commented I’ve so far been diagnosed for Anxiety, Depression, PTSD. However what’s really causing the main issues is still being discussed, including the talk of my constant hallucinations and audio hallucinations. Schizophrenia is out though because I’m too aware and it was crossed out pretty easily.

    that's why i dont't like people making threads out of threads of videos of people shouting xenophobic things in supermarkets. If you see some of those "famous" videos (not all of them obviously) the people in them have obviosuly psycological problems, and clearly complicated ones like psycosis or grave depression etc. So people talking about what they say instead of how, if the system would have helped them more ,those scenes wouldn't have happened, really makes me sad. Because it's even clear from what they say that they're just shouting random things. But Twitter does Twitter so,...PLus, a personal f*ck to all the people who record those things without thinking about talking tothe people involved through the situation and try to solve it

    considering this is an educational video, at least imo the picture chose for psychopath is v out of touch for a medical professional. u r still talking about a mental health condition, and the ppl inflicted w it still deserve respect. they r not inherently abusive or need to be locked up, n stuff like this perpetuates that stigma.

    This sounds like my sister my mom has lost everything because of my sister yet she'd fight you and kick you out defending my sister. And my sister will tell everyone she's taking care of my mom. My mom hasn't stopped babying my sister who just turned 40 ever I her entire life. My mom's and my sisters entire life. If my sister tells me mom to do something she'll do it even if it's don't go visit your grandchildren. Don't buy yourself this house cause it doesn't suit my sister

      Generally we think of fears as anxiety or phobias. But a psychotic person can be fearful. Fear is not a diagnostic criteria.
