I have no idea what OP is talking about. You are correct, it is the Cascade rune (bottom left). Every third shot has increased size, damage, and infinite pierce. Default mag size is 6, hence 4 normal and 2 cascaded rounds. Cascade also affects extra bullets as separate entities, like the third volley of Fan Fire will all be Cascade rounds.
