  • The National Transportation Safety Board (NTSB) will conduct a two-day hearing in Washington, D.C., to look into the events surrounding the January 5 loss of a door plug from a Boeing 737 Max 9.
    The aircraft, operating as Alaska Airlines Flight 1282, had just taken off from Portland International Airport (KPDX) in Oregon, and as it passed through 16,000 feet, the door plug blew off, resulting in rapid decompression. All 177 people on board the aircraft survived, although eight were injured by the loss of pressure that resulted in loose objects and parts of the cabin interior being turned into projectiles.

    I counted several unsats just on the photos provided of the door assy. Didn’t see any inspection lacquer on any mating brackets or fasteners Bad policy. Also paint was used to fill in gaps

    My solution is have USAF perform oversight on pressure fuselage parts prior to final closure and test

    Some were the risk assessment was missed and something wasn't checked on the airplane to make sure its safe to fly. Steps need to be taken and airplane need to be checked more frequently and properly...so people feel safe they want to fly .

    There should be a rip out procedure that reinvokes inspections when a door is removed through testing

    Pressure boundary inspections should be witnessed at installation and documented with procedural step b y step compliance

    The problems were pointed out decades ago when Boeing was caught using an outsourced company in Central America that cut the fuselage parts without using CNC machines, they were using magic markers and rulers. The door have to be fixed because the assemblies are not correct the are all upstream issues so they can "save money" not because people on the floor didn't sign paperwork. There should have been no need of "fixing" anything they should have rejected it and out those doors with wring rivets or placements in the scrap heap. A real quality system and a real CEO behind them is what they need, not excuses.

    bpi 1581 is ridiculously complex, like it was written by lawyers instead of by people that could convey the bpi in laymen terms to shop floor personnel. And this is part of the problem.

    In this presentation, it is unfortunate that viewgraphs and videos are not shown to the public because they would facilitate the understanding of the various elements. I hope in future presentations this will be remedied. Also, I think the names of the names of the affected "companies" be added to the names of their personnel to establish continueous relationships.

    This is more of a malpractice that is criminal. Falsification of documentation and failure to follow established policies and procedures.

    I also believe alot of pilots needs to start doing old school.check themselves not just replying on all gadgets in a airplane and auto pilots .Manual flights , manual flight computers, manual checklists outside the airplane and inside

    Staff back m rounds and mechanical check history on the airplane and pick up on the down falls and correct them ...rather than blaming each others and questioning all the times ..something news channels need to get grips with.🫡

    Admit faults ..fix them straight away... make sure dont happen again ...and move forward...this int no comedy show its serious business that needs to be addressed and fixed done problems solved!🫡
