    I'm really glad I stumbled upon your video! Very informative! Questions though,
    1. You said you build and design order sets and interventions....how? Do you program or reprogram existing software? What programming language?
    2. You got epic training from your organization and that was a must. If a hospital uses Cerner, would the expectations be the same as Epic? Must become certified?

    • @briankfung
      @briankfung  6 лет назад +2

      Hey Solo B, thanks for watching and leaving some comments! In regards to your questions,
      1. It actually depends on your EHR vendor. Though, there is usually minimal programming or coding required to "build". If you've ever built any type of online/electronic form (e.g. google forms), it's kind of similar in that you kind of just group content together and present it to end users. I'm hoping to do some videos that explain "build" better because it's both a common and excellent question. The most 'coding' I've had to do was when I was "building" alerts/pop-ups for providers in McKesson's HMM system. I had to build the colors, layout, & text in HTML.
      2. In my experience using Allscripts, Cerner, McKesson, and Epic, I haven't had to go through as formal or standard type of training as Epic. However, there is usually some type of 'training' that is provided. Though, I'm not sure how rigorous it is because you literally can't access Epic without first getting 'training'/certified. From word of mouth for Cerner, you had to do 'training' as well prior to access. Hope that helps!

    Thank you for the video Brian very helpful! Im on a pharmacy informatics APPE and frankly the department is extremely busy with integrating epic into a hospital outpatient pharmacy and a new independent hospital to a point where I feel like im not learning much even though there is so much to learn.
    What are some major controversial or simply important topics in Pharmacy informatics that are hot now? This could be related to CDS and smartpumps, alert fatigue. I dont have any good references to master these concepts and how they are applied in a hospital setting. I appreciate your help.

      I’m sorry to hear that you’re not having a good experience so far. Unfortunately, implementations usually make it quite busy for everyone at the hospital so I can only imagine how your APPE is going. The hottest topics in informatics right now are 1) artificial intelligence and it’s subcategories like machine learning and natural language processing, 2) population health informatics, 3) telemedicine, 4) the new interoperability rules from HHS/ONC which are expected to finalize in February 2020

    If I have worked in retail Pharmacy for 18 years, and burned out by the pressure of having to dispense medications more quickly and still safely. At the same time, customer service is a constant priority. Trying to manage those two aspects is never easy, but is becoming more burdensome in the corporate retail model. Do you feel that Pharmacy Informatics is a possible new course, with growing job opportunities, for a person who is interested in that field? If I’ve been in retail only, would it be better to get certification in Pharmacotherapy first?

    • @briankfung
      @briankfung  4 года назад +1

      Hi Paul - I've only experienced 3 months of the community pharmacy world and it was absolutely draining. Props for sticking it out for that long as it's a great service for the public. As to your question about pharmacy informatics, I actually don't think the job market is increasing. If anything it has decreased relative to the "Meaningful Use" era of 2009 - ~2017 when everyone was implementing electronic health records and the demand was high. Most organizations already have an EHR nowadays so I'd say the job market has decreased relative to the last decade and is likely more in a neutral state right now. As for getting BCPS, I think it depends on what role you're looking for. If we use informatics as an example here, BCPS is likely more beneficial for clinical informatics roles vs. operational informatics roles, relatively speaking. Moreover, I'd probably say the certifications and/or degrees you pursue will more largely be dictated by the job qualifications - which are extremely different across informatics. If informatics is something you're actually considering, I'd probably suggest 1) learning about informatics and figuring out if it is actually something you like and enjoy, 2) look for informatics roles that you are interested in - there are many. 3) base your next steps on #1 and 2 in terms of who you should be networking with and what formal education, if any, you should pursue. Hope tha thelps!

      Hey Risa! To your first question, general salary is probably equivalent to the typical pharmacist salary in that geographical location and usually slightly higher than your clinical counterparts since it's usually an admin/management type position. To your second question, you can certainly get into pharmacy informatics without residency. I made a video about it here that may be helpful! ruclips.net/video/mxfgmsCVBXY/видео.html

    For the bug drug mismatch. Is this a report the physician gets daily or a dashboard accessible for those specific physicians can access via EPIC? Sentri 7 was a program I've used to help pharmacist focus on certain issues (antibiotics stewardship, INRs, CrCl, etc). Despite EPIC also a similar capabilities?

    • @briankfung
      @briankfung  7 лет назад +1

      wow, this was a very excellent question! For the bug-drug mismatch, we use Epic's scoring system tool to display a list of patients that are flagged as having met that conditions for that rule. It's not really a report, but rather a dynamic list that is updated in real time as new lab results come in. I haven't used Sentri 7, but it is extremely similar in terms of functionality. As of Epic 2014, they've completely revamped a lot of the tools for stewardship and pharmacy monitoring - main goal is to replace those third party vendors (sentry 7, theradoc, etc). So yes, Epic has similar capabilities - AND it's integrated!

      Hey Ruslan! I’m not as familiar with the job market in Kazakhstan, but if the hospitals in your country have implemented electronic health records, I would imagine there being jobs available. Epic is a major vendor in the US and they have expanded quite a bit to other parts of the world in Europe, Africa, etc. so if I were to guess, I’d say yes! Hope that helps!

    What is the difference between health infomatics and pharmacy infomatics? Can a pharmD graduate with a degree in health infomatics can take up pharmacy infomatics jobs?

    • @briankfung
      @briankfung  4 года назад

      Hi Sowmya! This is a great question. I'd actually say they are the same thing with pharmacy informatics being more specific as to what type of health informatics you're referring to. To be honest, the informatics community has continually defined and redefined these definitions all the time. I currently use the AMIA informatics definitions and how they group them. For that matter, pharmacy informatics is a subset of clinical informatics which is a subset of bioinformatics. www.amia.org/about-amia/science-informatics
      A pharmD graduate w/ a degree in health informatics can certainly take up jobs in pharmacy informatics. It really depends on the job though. A degree doesn't necessarily dictate a guaranteed position. Work experience is also quite important in these roles.

    What skill sets are required? Will doing a CISSP certification help in pharmacy informatics?

    • @briankfung
      @briankfung  3 года назад

      Hi Razia - that’s a very difficult question to answer. Pharmacy informatics itself is quite broad and the skill sets required differ depending on your role. With that said, there are certainly a broad set of skills that would be beneficial regardless of role. I think any certification you obtain would be beneficial vs detrimental, but the ones you obtain should ideally be catered to the specific job responsibilities of your job.

    Can you please tell me the difference between a Clinical Pharmacist and an Informatics Pharmacist? Do both these jobs require a Pharm.D? Does all Hospitals use these methods of Informatics Pharmacy? I hope you'd answer these questions. Thank You.

    • @briankfung
      @briankfung  6 лет назад +1

      Hey job1n! Great question - I'll try to be brief here. I'd consider any pharmacists that graduates with a PharmD (or previously with a BS) a 'clinical pharmacists'. With that said, I'd say an informatics pharmacist IS a clinical pharmacist and that informatics is a specialization of clinical pharmacy. If we take away the 'clinical' pieces of an informatics pharmacist - we'd be just technical people. The main difference can probably be summarized by saying that informatics pharmacists work more so as a liason between your dedicated clinical pharmacists and technology (e.g. EHR, ADCs, smartpumps).
      Prior to 2004, you didn't need a PharmD to be a pharmacist - so technically no. There are still pharmacists with a BS degree that is more than capable of either of these jobs. You can actually work in pharmacy informatics without a PharmD - but you wouldn't be an informatics pharmacist. There are many pharmacy technicians that are in pharmacy informatics as well.
      Yes, all hospitals and organizations in general utilize some method of pharmacy informatics. Informatics as a science is about communication of information - we're just doing it more electronically now than before due to the affordable care act of 2009.
      Longer than I wanted, but I hope it helps!

    • @briankfung
      @briankfung  3 года назад +1

      Take this with a grain of salt as i've never worked in the pharmaceutical industry. With that said, I don't think informatics is necessary, but rather beneficial to have, if one was to apply to the pharmaceutical industry. Even then, "informatics" is such a vague and overarching term that there is unlikely any value that can be derived from such a statement. My general thought would be that having a basic understanding of how data flows from system to system, especially in the contexts of how the pharmaceutical industry functions, will be beneficial.

    • @briankfung
      @briankfung  5 лет назад +1

      Hi Arnaud! There certainly is, however, it's more of a sub-specialty within pharmacy informatics. Much of pharmacy informatics can be described via the medication use process while data analysis/science is more of the quantitative side geared towards decision making (e.g. CDS, reporting). My personal opinion, pharmacy (and pharmacy informatics) as a whole is still in its infancy when it comes to data science

    • @briankfung
      @briankfung  7 лет назад +1

      Nope! No IT classes needed for my job! It sometimes is helpful to have some basic proficiency in excel or SQL, but it certainly is not required. I would say the majority of individuals I've worked with do NOT have that experience. I learned most of my skills during pharmacy school or residency training. Pharmacy informatics is very vendor-specific and most of my training has been with Epic. In order to even work with any organization that uses Epic, as an informatics pharmacist, you must take formalized training from Epic. Typically, your hiring organization pays for you if you don't have it. With that being said, the most important thing is to concentrate on your clinical training during pharmacy school.

    Hey Brian, great video! If you were interviewing someone for a pharmacist informatics position, what type of questions would you ask?

    • @briankfung
      @briankfung  5 лет назад

      Thanks Lawrence! In regards to your question, it'd probably be highly dependent on the position. However, at a high level, for specialized positions, I'd probably inquire about their previous experiences with that area (e.g. reporting, automation, smart pumps, med build, etc). I also ask questions that were actual problems that were both within and outside our organization (e.g. if our group of ED physicians wanted to turn on duplicate alerts for opioids, what would be your thoughts/next steps?).
      Aside from the specific questions above, general situational and/or behaviorial questions are important to gauge how they would function in the team. Questions that pertain to conflict (e.g. tell me about a time you had a conflict with a colleague on a project and how you handled it), time management (e.g. if you were balancing multiple projects and an earlier deadline was set for one of them, what factors and/or next steps would you use to evaluate your priorities?), etc.

      Hey Terrance! Great question - you don't actually. A big portion of the pharmacy informatics workforce are pharmacy technicians. And depending on which certification you'll looking for, you don't even need to be in the clinical world to obtain it. For example, Epic and HIMSS (e.g. CAHIMS and CPHIMS) you don't need to be clinically trained (e.g. pharmacist or pharmacy tech). I believe the new one from AMIA (i.e. AIIC), requires clinical training since it's directed towards clinical informatics. I'll have to wait until that certification is released to learn more though. Hey Khaled! There are many areas that would benefit from the expertise of a pharmacist with programming skills. However, the vast majority of the work we do in pharmacy informatics doesn't require programming skills. Having basic understanding of it certainly helps, but is definitely not needed. With that said, there are some areas in pharmacy informatics that would benefit more than others and the two that come to mind are 1) reporting and analytics - usually SQL and 2) clinical decision support - simple/traditional understanding of boolean logic (not programming), but starting to see more individuals dabble in SQL, Python, and R to help build algorithms.
