Severe Hyperkalemia Emergency

This patient with end stage renal disease and Goodpasture syndrome may have missed dialysis. She presented in extremis with a "sine wave" EKG consistent with hyperkalemia. Her EKG normalized in 3-5 minutes with insulin therapy followed by glucose. Calcium and bicarbonate were also given but the tightening of her QRS was due to the insulin therapy.
