I like that Sam compartmentalizes his inner thoughts by projecting specific arguments onto individuals from his life. And I appreciate this method of storytelling too, it can easily let the audience know Sam's inner thought process. By how Alastair, Young Sam, his mother Mary, and Dean speak about what issues, by what these people said out loud, it is how we know Sam feels and what/how he is thinking about himself.
This is such an iconic episode.. so much happened, another one that hits me like a full length movie. The hallucinations are nuts.. all over the map.. torture from Alistair, condemnation from his younger self.. pep talk from mom, flying around the room..Dean cutting ties...so many different messages. The kid playing young Sam.. so good. I liked having a couple of good Bobby moments.. Rufus too, even just on the phone. Moment of behind the scenes levity. After fight scenes like this, when they cut, the crew used to tease them with, "We hate it when they fight" because both of them would really get emotional with big ones.

We all hate it when the boys fight😔 on to the finale, where all your questions and more will be answered. In case you didn't know the last thing Dean said "When you walk out that door, don't you ever come back" was the last thing John said to Sam when he left for Stanford. A very sad parallel.
It may be a smaller thing to appreciate about the show, but I like how many episodes are named after classic rock songs. Especially when they use Led Zeppelin inspired titles, who are rightfully Dean's favorite band (though Supernatural didn't have the budget to license their music for use on the show).
The show's canon is that Sam's power is sourced from evil. Even a small amount of him giving into that power will change him, as every Special Children did. It's not like he can just use those powers for good and stay good. He WILL be corrupted morally and physically transformed like Cas said as he succumbs to it. Consuming demon blood changed him - his eyes turned demon black and his body got stronger. Ultimately his moral code. (Spoilers for 4.22, sorry Imon I know you've seen the next episode I have to take a point from that one to emphasize my point here). Sam killing the nurse is the last moral boundary to cross over to the other side. That is the reason Dean adamantly didn't want Sam to give into that whole shebang, he'd prefer Sam to die human when detoxing.

One of my least favourite episodes. Sam's suffering and pain is heartbreaking to watch, even if Jared's acting is marvellous. Of course the ambiguous Castiel betrays the brothers and keeps his mouth shut about it.
