  • Dr. Ebraheim’s educational animated video describes Hangman's fractures.
    Hangman's Fracture is a bilateral fracture of the pars interarticularis. The spinal canal is widened and there is a low risk for spinal cord injury. The fracture usually occurs more due to motor vehicle accidents.
    Anatomy
    •Spinous process
    •Body
    •Axis
    •Pars interarticularis
    Mechanism of injury
    Hyperextension will fracture the pars interarticularis with secondary flexion injury the disc and posterior ligament. The patient may have other associated spine fractures (up to 30%).
    Levine and Edwards classification
    Type I
    •Stable fracture with less than 3 mm displacement
    •No angulation
    •Treatment: cervical orthosis
    Type II
    •Most common type
    •Significant translation and some angulation
    •Unstable fracture
    •Treatment: cervical traction to improve the displacement and immobilization in halo vest.
    Typer IIa
    •Slight translation but severe angulation seen in flexion-distraction injuries with tearing of the posterior longitudinal ligament and the disc. The fracture is unstable.
    •Treatment: reduction in extension and compression in a Halo. Do not use traction when there is severe angulation of the fracture.
    Type III C2-C3 facet dislocation
    •Rare fracture that results from initial anterior facet dislocation of C2 on C3 followed by extension injury fracturing the neural arch. Results in translation with unilateral or bilateral facet dislocation of C2-C3 unstable fracture.
    •Treatment : Surgery for reduction of the facet dislocation and stabilization of the injury
    Typical and atypical fractures
    •A typical hangman’s fracture displaces the vertebral body anteriorly and its posterior element posteriorly. This creates increased space for the spinal cord.
    •An atypical hangman’s fracture line leaves the canal circumferentially intact, which puts the spinal cord at risk of injury if displacement occurs.

    I was in a car wreck and broke c2 and c3 with fractures to c1 and c4. That was 5 years ago, I'm now back at work and putting about 70 hours a week in. So far the only issue is numbness in both feet can't bend my toes, and sensitivity issues. Thank you to all people who become doctors and help people like me.

      MY NEPHEW 21 Y OLD HAD C2 VERTIBRE problum he can hold his head now he is on bed rest with colar

      I had two fractures in C1 and C2. I did not have any operation, just took bed rest and medicine. The doctor said that it is very risky. And I was cured and then fractured again the day before. I want to fix this , my 2 years are useless .

      Did thay tell you how to thin the blood out if you feel pain in your right ankle you need to drink alcohol to thin your blood out as the injury pressurized the scull you should also be on opiates to slow your digestion down and stops the smell of undigested food when you pass wind also you should get calcium tablets and mix milk in till it looks like thick sludge then you lie on an angle with your head at the bottom of a slide and swallow a bit of the mixture the bones will soak it up in the neck or use powdered bones rams horns thay also work then your neck can't brake also valaran root if your lips feel warm the nhs dont tell you this ive got a hangmans fracture from child hood a doctor who is retired told me how to survive it look after your heart and you will live for years on end .

    I had a C2 fracture & survived. Tripped in my own bedroom & grabbed a bedpost & "whipped" my neck. The. factured bone compressed an artery that goes to the cerebellum & thus caused a small stroke that affected my balance. .I walked around for 4 days before I went to a doctor who took an Xray & sent me ASAP to hospital. It was then discovered about the blood loss to the brain & my balance began to malfunction. I was in trauma center for 3 days & in hospital rehab to restore balance for 2 weeks. I had no surgery. I got the neck brace off in 6 weeks. The fact that I am alive & not paralyzed is truly a miracle. I send my miracle to all who read this.I survived but I seriously don't now why. I send all of you luck & love that survive this injury.

      I type this with tears in my eyes, I survived a c2 fracture and is currently is therapy and not paralyzed. I thank God soo much and your comment just reminded me how good he is!

      @@daKingCainwayTV Same here I thank God I'm still alive, I broke or fractured my C2 6 weeks ago in a car accident and survived but in a lot of pain but not paralyzed, I'm worried to death though I move wrong and get paralyzed, I'm wearing a hard collar brace for 6 weeks now. I just got an xray on a disc 4 days ago to get ready to take to the DR so he can see it and was just looking at it on my CP laptop and it looks like it's still broke cause it looks just like the one on the video. Good luck may God and Jesus bless and heal you.

    Thank you for making this video. I have this type of fracture, and I didn't really understand where the bone had broken. I was having a hard time picturing it or understanding completely. I feel like I understand it much better now and I feel more confident and comfortable about the treatment and less scared of what might happen as I'm healing. This is such a valuable resource you have created. It's clear and easy to understand.

      I fractured c2 3 weeks ago can I carefully massage my neck ? Feels good to do it but I’m scared ima injure myself more ?

      @@comptoncarlos1726 I see passed it over 1 year now. So I wonder do you feel healthy after treatment? And most importantly can you turn your head left right fully? Is there maneuver restriction in your neck? Are you totally helpful?

    I broke or fractured my C2 and C4 vertebrae 6 weeks ago in a bad car accident and it still hasn't healed, I'm wearing a hard collar neck brace and in a lot of pain, I got an xray on 1/25/22 on a disc ready to take to the neurosurgeon Dr to see what he says, I looked at it on my CP laptop and it looks like it's still broke or fractured because it looks like what was shown on the video. I'm worried to death I'm going to die the internet says a C2 break is very rare accounting for only 2% of spinal injuries each year they are difficult to treat and are the worst spinal cord injuries a person can receive and often fatal. This was a good video and made things clear a Hangman's fracture I wonder why they didn't tell me that at the hospital emergency room? No wonder the DR couldn't look me in the face.

    I have c2 fractured now! It's really stressing. It's almost 3weeks when i was slid. Wearing hard collar neckbrace is really hard.

    My large dog tripped me at the top of my stairs. I did a header to the bottom and broke C1 and C2. I'm a freak of nature. Nothing wrong with me and just 6 weeks in the damn hard neck brace.

      @@priyanshmuyal9750 That sucks. Maybe ask for a second opinion and get another CAT Scan done. I'd suggest the torture device of the hard brace for a while. It's like a cast for the neck. There's a worse one, but they drill into you for that. Take lots of calcium and drink milk. Otherwise, treat your neck like glass. Good luck.

    About 2 years ago my brother strangled me with all his force, he is about 100kg so its quite alot. Since then i couldnt move my head for 2 months and afterwards it feels like my neck is pushing into my adams apple and i cant sleep properly.

      If it's not a real bad fracture 4 to 6 weeks if you're careful maybe 8 weeks if it's worse.

      @@comptoncarlos1726 I hear you bro I'm in the same boat it's been a little over 6 weeks since the accident and my neck still hasn't healed and hurts like hell and haven't been treated yet, they didn't pick me up to see the DR last week and I missed my appointment. Just be careful and hang in there. Look on RUclips and type in C2 vertebrae The Hangmans Break.

    I fractured c2 3 weeks ago I noticed at the end of the day I’m in a lot of pain obviously because I’ve been up all day trying to do things that have to b done my question is can I massage my neck or have my girl massage my neck as long as we’re carefully massaging it ?

      I'm sorry no one replies to you, I would tell you but don't know if it's ok to massage or not. I broke my C2 vertebrae in a car accident 6 weeks ago and worried I might die, the internet says breaking the C2 is very rare accounting for 2% of all spinal cord injuries each year and the worst spinal injury a person can receive and the most difficult to treat sustain and often fatal, you should go to a DR and ask him don't take no chances.

    I believe type III is the actual fracture you'd get if you were long drop hanged, since the name of this fracture.

      this one is mostly a hyperextension fracture, whereas in movies you often see them breaking the neck sideways
