    Hey Neil. I watched your series and wanted to extend my thanks for making it available. I go in for my TRUS in a couple days. I'm not yet even fifty, and this whole prostate business (right now, mostly the biopsy) is really twisting my brain. On one hand, I want to know the results and want to get cracking on addressing the cause of my elevated PSA, but I'm also fearful of having do deal with the range of possible outcomes in that address. The biopsy is like the first step on a journey I'd rather not be making, frankly, but my feet are already on the path and I've been looking for little bits of courage. I've found a few here, from you. These little films of yours have been very helpful. Thanks again, and I'm hoping the best for you.

      Hi Randy - sorry i didn't reply to you 2 years ago - I missed your comment.. I hope it went well for you and that you are now in good health. It is now 2 years since my robotic prostatectomy and I'm totally back to normal with no issues at all.

      I had the TRUS done also in the urologists office. I was apprehensive to say the least. I assume the ultrasound nurse gave the numbing shots, but they were painless. The urologist came in, and they discussed the procedure with each other, when I heard the gun snap. I felt nothing. The numbing shots were properly positioned apparently. I was in and out of the office in 30 minutes.
      Biopsy came back positive, and I had the prostate removed. The pathology report came back that it was bulging are ready to spread.

    I just had the Biopsy last Tuesday, now waiting results. I'm an electrical Engineer, so actually having another Engineer explain it all works well for me. Thank for that. With respect to the Biopsy, my issue was the dressing they used, it was stuck down really hard and getting it off hurt worse than anything else, and risked upsetting the fresh wound.

    Thank you for posting your experience. I had a MRI fusion transperineal prostate biopsy last Tuesday in Lisbon. In my case, the surgeon inserted the catheter at the end of the biopsy and while I was under the effect of the general anesthesia. The catheter was removed next morning and after that i could not pee. But after three attempts during one hour, i was able to pee again but with a lot of pain. The pain remained until today. The other side effect is blood in the urine. Apart from these side effects, i feel much better than I felt after the TRUS that I had last year.

    I had mine under local anaesthesia. The first biopsy needle was painful and so the doctor gave me some more local anaesthetic. It was painless afterwards. Ideally, the doctor should wait 2 minutes for the local anaesthetic to take effect before starting the procedure. I would advice anyone having a transperineal biopsy to take painkiller tablets immediately after the procedure because you start to feel some pain about one hour after the procedure when the local anaesthetic starts to wear off.

    Neil, thanks for this & for your previous video on your transrectal biopsy. My TRUS is scheduled in 6 days, & I'm greatly comforted by your own experience that it was mildly uncomfortable at worst. I'm glad you're up & about, & I hope you get the best results. Saul S.

      Hi Saul,
      Its now been 6 days since the procedure. The biggest effect was certainly the anesthetic, & the biopsy site discomfort was trivial. I'm still with a catheter and been at work for 3 days, and feel fine. Any catheter discomfort has now disappeared, and today, Sunday I've been working on my hobby lathe all day, and forgotten its there. It comes out tomorrow.

      UPDATE: Neil, it's been 2 days since my transrectal biopsy, with no worrisome after effects. As you said in your first video, the procedure was no more painful than 12 rubber band snaps, if that. I'll get the results on March 14th (the Ides of March minus one!). Hoping for continued good news from your quarter. Saul

      Neil I had my Transrectal biopsy yesterday. I felt great comfort from hearing your words and experience that you went thru. I'm 53 and been dealing with an enlarged prostate for a long time. But just recently my PSA levels went from 2 to 6 back to 4 then 4.7 thats when my doctor told me it's time. I thought of you when I laid on table about the rubber band snap you mentioned. That really didn't bother me it was the probe in my butt I guess pushing on my prostate. Other then that not bad at all. Thank for posting your video's I get my results next week.     Thanks for sharing your story. Really appreciate it. Admire your honesty and easy simplification of the procedures. I'm an Aussie almost 66. I had an MRI scan done a week ago and it picked up a small 3mm lesion rated PiRad 4 which means a possible biopsy. My last psa just prior to MRI was 4.1. Seeing specialist in a week's time. Hope your ok. I haven't yet watched your prostate surgery videos yet but I intend to do so

      Thanks for the comments Tony. I do intend to do a followup because its been a while ( like 5 yrs) since the surgery, but it went well and I am 100% now; psa = 0, and no side effects or after effects. I was back at work a week or so after the surgery.

      @@neil1058 Hi again Neil. I'm very pleased and happy for you that everything has gone so well for you with the operation and your recovery. I'm hoping for similar outcomes of course. My psa was 4.1 prior to my MRI scan and I'm due to see a private specialist next Monday 12th June to discuss my mri results and I would imagine discussing a biopsy as soon as I can so to ascertain if it's cancerous or benign and then what to do if cancerous. I know there are so many options with either robotic surgery or radiation and it's difficult to determine exactly the best option. My main concerns after either is incontinence and not being able to live a normal life like exercising, jogging, bush walking etc....anyway thanks for sharing your story. It was very encouraging and informative

    General anesthetic !!!! you lucky bugger I had this done recently on a local anesthetic on the UK NHS (these people are the best nicest caring people you will ever meet) but I didn't get stockings either just a gown that didn't tie at the back, I was given enough local anesthetic for a person of 70kg but I weigh 108kg after about 10 biopsies they realised I was bathed in sweat and was crying with the pain so they gave me extra local (for which I was very grateful) and then they carried on, I lost count of how many they took but each one was painful, though not as much as the first batch . The lowest moment was when they finished and I had to stand up and walk back into the other room to get dressed, as I stood to get out of the stirrup chair I started to fall forward, the nurse held onto me and pushed me back onto the chair during which my gown came off completely and as I lay there naked I could feel the blood dripping off my bum cheeks onto the floor but 20 minutes later I was driving home, what a day.

      Hi Martin thats not a pleasant experience. Glad you were able to drive home though. I didnt get to go home until the next day. Its all in the past for me now, with a successful prostatectomy after the biopsy. its now a few yrs later and no side effects at all.

    I had mine a couple of days ago - I seem to have got off lightly compared with you. No Sickness - No Blockage - Little pain. Another two weeks for my results - but with a "real" PSA of 11+ (I am on Finasteride) I am prepared for the worst. Hope you are well Neil.

      Good luck with the result Russel. Its now 18mnths since my prostatectomy and all is well. PSA of zero so far.

      What was PSA, was it perineum biopsy and how many core samples?

      Im about to have one under local, cant say Im looking forward to it, good to hear somebody positive about the procedure thank you. My PSA was at 6 and then I had another test some months later and it was back to normal so who knows but if the doctors finger said it didnt feel right I guess I need to go through with it.

    When i had a biopsy and cystoscopy done at the same time, I don't know why I was not put to sleep. I went through alot of pain. My next one is in 6mos. I going asked to be put to sleep!!!!! Yes, you do need that support!!

      Was this a transperineal biopsy or TRUS? Im guessing it was the cystoscope that was painful. I probably would have asked for light sedation.

      @@9neilIt was trans perineal biopsy along with the cystoscopy. I was lightly sedated for the biopsy only.

      And you never want the pleasure from neither without general anesthesia!!! Unbelievably painful. I don't know why I was not put to sleep. I was numbed but not put to sleep. The number procedure was painful as well for both!!

    Great Video, This happened to me, except in the States you get this in a Clinic. It felt like 17 Bee Stings, then if you can't pee, in goes the Catheter. Go home with strong Antibiotics with a warning don't lift anything heavy, you can rip your mussels. I had my wife who is nurse take out the catheter in 2 days. Then make sure you can pee or back to the Doctor.🐡

    Thank you for your direct explanation, Im about to have the same thing and I am very uneasy about it so I appreciate your honesty. My worry is from the indications I have so far is mine is to be done under local which really doesn't fill me with excitement, I could be wrong, I hope so as I would rather not be there during the procedure and just address the end results ( forgive the pun ) .

      If you are not happy with a local, its your body - Id insist on sedation, or whatever you are comfortable with. In my country, its my right.

    Well,Neil,all i can say is that my experience of a biopsy was totally different,here in Cork, Ireland. Mine was done over local anaesthetic and overall was pretty awful. Firstly,i was given local anaesthetic up the perineum. Then followed four long core needles, one of which really hurt. The camera probe up back passage was very uncomfortabe and found it difficult to sit down for couple days! Nearly three weeks later, I still pass blood in my urine. I did ring the hospital...nurse said keep drinking plenty of water! I am just dreading any treatments i have to contend with as its 99 percent sure its cancer

      Hi Gerald,
      I'm sorry to hear of your experience, and i'm surprised that a transperineal biopsy was done under a local, and not a general or least with midazolam. Its all about the cost! (Midazolam is an IV tranquilizer and is safe and is magic stuff , in that there is no memory of the experience at all. I've had it 3 times now for other procedures, and have no memory of any pain or unpleasantness at all.) Passing pink urine for a couple of weeks after happened to me but was not an issue.
      If it is cancer I hope you have caught it early, and all the best with the future. ( good rugby games you guys played recently. Well done !!)

      My first biopsy was back in 2014 was done transrectal. Blood in the urine for over a month and in the semen for months. Then 45 days of radiation therapy. PSA remained low (.64) for about six years and then started to rise. PSA got up to 10 and I had another biopsy, transperineal done under anesthesia. Then I had cryotherapy. Last week I had my third biopsy. This time transperineal under local. And man did that ever hurt. Those needles were about 10 to 12 inches long. About six needles to numb you and then twelve biopsy needles. But as painful as it was, it was quicker, had minimal prep, and so far recovery seems quite a bit better. Haven't talked to the doctor yet, but have seen the pathology report. Many areas that had shown cancer before now are clear. However, one area that had shown no cancer shows perineural invasion.

      ​@@neil1058Thanks for that Neil. Sorry to hear of your bad experience.....so different from mine,altho it was an unpleasant experience too. Diagnosed gleason 3+4,with psa in the thirties. Put on Bicalutimide capsules which caused palpitations....as a result, i was taken off those and put on monthly injections of Firmagon,rather than the usual three monthly ones. These cause frequent hot flashes which are particularly debilittating in a warm environment or on a warm sunny day. The radiotherapy itself was fine except for bowel problems. PSA now down to .045 which is good news...now if only i could come off that Firmagon. Prescribed for two years,I may get off with 18 months as doing so well.
      Looking forward to beating you guys again,perhaps in this years World cup,and maybe even in the Ladies football World cup,eventho you guys are rated above the Girls in green

      All the best John. Please report back. The worst side effect I had was some nausea afterwards. There was no pain that was significant.
      I ended up having a robotic prostatectomy a month or so afterwards ( which i have posts on) , and likewise - that was pretty easy and I was back at work a week later.
      Its now a year on, and all systems are good, and it is no a distant memory with no after effects. The secret - if surgery is indicated - have the robotic surgery and find a good experienced surgeon.

    An interesting video Neil - I have mine coming up in a couple of weeks - I'm having a "targeted" biopsy - which I think means only a specific region (anteria with me) of the prostate is sampled. Hope I can avoid the catheters !

    Fellow Aussie. Australian Capital Territory. Ten minutes, plus 5 minutes resting afterwards. Aged 71, generally healthy & drug-free. PSA about 12 for a few years. Urinary incontinence hence this procedure, and the MRI of the prostate in a few days.
    Few local injections of local anaesthetic into the perineum, plus one precautionary antibiotic injection into the thigh.
    Could observe the ultrasound on the portable screen near my waist, if wished. Black & white video, but made nothing to myself. Would have preferred listening to distracting music on earbuds. Similar to visiting the dentist.
    Quickly returned home. Spots of blood in urine & from the anus for first 2 days. Nothing alarming. Now the process seems simple, quick & low cost. Luckily our Australian Federal government covers all my medical expenses now. Aged Care plan.

    Thank you for making the videos. Lignocaine with adrenaline anaesthetic for skin infiltration and lignocaine for infiltration above the pelvic floor muscle either side of the prostate under ultra sound visualisation, is mentioned in other videos; and with my wife having been given Entenox during her colonoscopy -as she wanted to drive home by herself so no sedation, leads me to ask the question; could the nausea of the general anesthetic be avoided, if men were allowed Entenox during the trans-cutaneous prosatatic biopsies? When she sucked hard on the Entenox, the pain resolved - so synchronising inhalation of Entenox with firing of the needle biopsy, might enable this procedure to be done under local? Would you be able to ask your consultant about whether Entenox has been tried? Thank you.

      Hello Zenko. Its 2 -3 yrs now since that biopsy. Ive since had a Da Vinci prostatectomy and at that time I expressed concern about post op nausea with the anesthetist and he used an intra-venous anesthetic in lieu of gaseous. I had no nausea at all.
      The procedure went very well - I talk about the process in another series of videos. It is now 2 or more yrs since that event and i have a psa of 0, and no after effects at all.

      I was back at work 2 days later With no significant after effects.
