He shows his second form if you got the complete resist ending, meaning you destroyed all elder shards. If I remember correctly, you can start the boss without having all of them, so you might be missing some

Man i compared the two endings you get for embracing and resisting and the embrace one is way too cool for the morally bankrupt option xD
but to me, the embrace final boss is way ridiculous harder than the resist boss for me.

How did you unlock 4th perk slot?
Its one of the perks you get for destroying the elder shards in the incinerator, the 5th or 6th one.
