I have a different question. My printer didn't print the shades of gray, basically everything that's meant to be blue but on your form is gray, it only printed what isn't blue but what is black.. does that matter? Because it's just shaded background

I'm sure it won't hurt if you included commas, they'll understand how it's supposed to be written. But definitely double check your data when you get passport back to make sure it's all accurate 🙌

You can leave out any info that doesn't apply. And for any info that doesn't fit, you can attach an additional sheet of paper and include that info there

Edit: State Department got back to me. They want (4) staples close to the edge vertical (2) on the left side and (2) on the right side. The forums shows where the staple go. If you use paper clip or some other sort of other methods. The photo will get lost and there going to be delay's.

And section next to picture name as it appears on current passport... your presentation shows First name first, middle then last, thank you, because my passport shows last name first....
The top right box is office-use, so no need to do anything with that. For your name just use the template that I described here because my last name appears on top of my given name on my passport as well

The colors don’t matter too much as long as everything is clear and legible. What your write should be only in black, however.

Yes you should be able to save a copy. You can sign it by hand once you’re ready to turn it in. Black ink only 👍

student would be okay, unemployed would be fine...I dont think that one matters too much because it's not info they include on your passport

Correct. You might get lucky if you’re writing is extremely legible, but block capital letters are what they ask for
