Something that has frustrated me often in learning coding is that the way it might be presented, I am unable to figure out why I got an answer wrong. You make things very clear when giving the explanation. I always say to myself, "Oh! It is multiple choice question exam. The test author doesn't expect you to spend much time getting into the detail due to time constraints. That said, use your common sense and follow my backward approach you should be able to narrow down the correct answer. You can do it. I wish you the best of luck for your exam.

Upper and lower lids are two separate lids so 67801 (multiple, same lid) is not the right code to use. If four chalazions are on the right upper lid OR right lower lid only then you can use 67801.

In the question, did you see the key word "complex" at the end? and in the CPT 66982 description, did you see the key word "complex" at then end? so why you choose 66984?
When you take the CCS exam, you will have to fill in the blank when you are coding cases so you have to know how to look up CPT procedure codes. Also, in the real world of coding, they don't give you multiple codes to choose, you have to read the procedure report and assign the right code. As I said in the comment section what you learn from this video not only prepares you for the CPC, COC, CCS and CCS-P exams but also for the real world coding practices. I hope it helps.

Yes you are correct. Orbital fat and/or excessive redundant skin removal must be documented in order for us to use 15823. I hope it helps.
