So, do varicoceles cause infertility? The answer is much more complex than you think. Varicoceles for long have been associated with low sperm count, low sperm motility and a host of seminal parameter impairments. Today, a large number of fertility specialists and urologists actively advocate varicocele surgery for a range of male infertility issues. It is indeed saddening to see this. The clinical evidence is quite clear, in the sense, that varicocele surgeries do not cause infertility issues. Moreover, clinical pregnancy rates after varicocele surgeries do not increase anyway. Subtle improvement in semen parameters after varicocele surgeries does not justify doing the varicocele surgery.

Hello doc, I was diagnosed with left sided varicocele grade 2 varicocele 10 months ago in September 2022 but in January 2023 right sided grade 1 poped out too, asthenoszoopermia was also diagnosed in the march so my surgery was done 3 months ago in april through cdvl technique where surgeon has removed my cremastic muscle compartment which cause tortoisity in the veins and ligated the pathological veins
But now I can see little dilation of 2-3 veins in the middle side and 1-2 in the lower side of testis, i don't know if this is due to redirection of blood flow to new veins or god forbid due to recurrence?
I heard from a video of dr raman tanwar urologist gurgaon that there is a new research that dilation of external spermatic veins scrotal veins gubernacular veins won't increase testicular temperature and cause harm to testis and fertility i heard it is like regular veins of hands and arms of bodybuilders, it happens due to changes in circulation and anatomy due to surgery but some surgeon even deliver testis and ligate those veins, please tell me what is the truth...

My husband is detected with a grade 3 in the left and grade 2 in the right, he visited an experienced urologist in Holy Family hospital bandra, he was recommended to fix it a surgery would be recommended, does he really need to go through a surgery? Did you have pain in varicocele and what was the vein size in mm.
Better, if u go to a laboratory, and take a semen analysis test, it will clear you whether you're fertile or not. The test result will tell you about your sperm count, motility etc.
But as your age is not 18 or 18+, it will be tough i think so. Don't worry about it. Focus on your study. Not on this things. If you continuous feels pain or something abnormalities in your scrotum you need to consult with the doctor again.

Sir, would 4 millimeter varicocele be a problem? Past 3 months now it's causing some pain, would you say the pain will subside when it settles down. Also it has raised my PSA levels. Have started using Horse Chestnut supplement. Would surgery be the last option only if there is constant pain?
If constant pain is there, surgery would be the only option. However. Post surgery, there is no guarantee that the pain would disappear

I don't know how does your program university allows to enroll into andrology without being urologist at first. It doesnt happen in Fundació Puigvert one of the biggest and oldest places to have training as andrologist, or any other serious university program admiting fellows within the US, England... You are mispreading wrong information among these patients looking for a solution to their problems. How can You dare to recomend patients to not operate when they are contacting You saying they have atrophy, low testosterone, ED problems among young man, how do You dare. I'm serious urologist with experience and trained in Microsurgery with thousands of patients treated succesfully. Better You recomend these patients to look for a good urologist with experience who will preserve Testicular arteries and remove all the veins to avoid artery damage and recurrences and more than 95% of these patients Will improve.

@@tseries5343 i guess even more. Last months ive noticed increasing sexual issues among younger patients, the only one fact that shows up at exploration and US it's varicocele. Couple weeks ago i performed operation on a 25 yo guy who was already under Sildenafil due to lack of firm erections. One interesting fact is that these patients has no longer morning woods or they are appearing less time after time. So they come to urologist appointment and the urologist does not know how to treat or what to do to help young men improving their erections. There are running at this moment a lot of investigation about varicocele and ED. In my experience over more than 2000 patients treated by Microsurgery, with microdoppler vascular device, with artery and lymphatics preservation including testicle deliver and resection of the group of veins, they are showing amazing results, consistent and reproductibles among young man. We are just few urologist treating on this way.

Embolization?
Oh no!! Not embolization or varicocele surgery required!!
Embolization only works if You have no Big external spermatic veins or gubernacular veins otherwise you will experiment relapse. Embolization does not reach out to the external spermatic veins neither gubernicular ones, so it really wont solve the problem in a big amount of patients. If you like to do sports or lift heavy weights then your blood will push on external and gubernicular veins wich leads to recurrence. Remember that embolization only treats internal plexus. Embolization recurrence is much higher than shown on the articles, same happens with the laparoscopic approach. The procedure that offers the higher success rate is the Microsurgery-with artery and lymphatics preservation including testicle deliver and use of micro vascular doppler device during surgery and removal of three groups of veins and letting only the deferential vein for blood return. This surgery must be done by urologist expert in Microsurgery so you can have the warranty that will be done properly and recurrence rates are less than 1% if done in this way.
