You missed one, and a cheaper option than most- the Korg SQ-1. Primarily a sequencer, it can nevertheless convert midi over usb to cv and gate. I use it a lot with the modular rig.
The best entry level recommendation is missing: Most semi-modular synthesizers with midi input also have Midi-to-CV conversion, like Cre8Audio East Beast/West Pest, Moog Mother 32, or Make Noise 0-Coast.

Cv.ocd and its daughter, mutant brain, supply 4 CV and a bunch of configurable gate outputs, for a very, very reasonable price. I also like the look of midi to CV modules from Flame.
