It happens sometimes, I think you might be releasing RT too soon and the game is just wonky. Try holding RT mindfully the whole time without letting go.

The controls of the combos work on any guard build, its just specific animations are better/faster.

Same . I do a size up first and the escape comes out after for whatever reason
Its the angle your flicking the right stick, do it more horizontal than vertical if that makes sense, like a 15-30 degree angle not 45
