Intraocular Lens Implant Options for Cataract Surgery | A Comprehensive Guide on How To Choose

  • THIS is what I tell my patients with cataracts and I would like to share my expertise with you today! The difference between a happy post-op patient and one who's not is often simply due to lack of communication between the eye doctor and the patient regarding lens implant options and realistic expectations.
    In this step-by-step video on the topic, I will explain the following:
    - What a cataract is,
    - How to know when surgery is recommended,
    - What intraocular lens implant options are available,
    - How to choose which one will work best for you

    I have no problems by wearing glasses. So i choose the monofocal lens because I think it has the sharpest view.

    I have done much research on about intraocular lens and how vision will effects after surgery but I did not found what it I expected.

    I had cataract surgery in both eyes on Monday February 6th. 2023 the same day. I was very happy. I had monofocal lens implants. I need prescription reading glasses.

      Hi. I am going to have cataracts surgery too. I can’t make a decision if I should pick laser or manual surgery. Do you mind let me know which one did you use for your surgery. I am living in Toronto too. It costs a lot for laser operation. Thanks in advance.

    I had the new Vivity multi-focal lenses inserted in both eyes and now have 20/20 or better vision from the small numbers on my smart phone to the horizon - without glasses. This new refractive lens also seems to completely eliminate halos. You may want to amend your very good video to point that out. The only negative is the very high cost for this option.

      Exactly agree. Had the Alcon vivity put in one eye with a bad cataract. Day 2, saw some halos in kitchen. Now, 2 weeks later, vision is perfect throughout all ranges with no halos, blurriness, except for very close up, might require cheaters. My near vision after one week was 20/35, same mid, 20/20 distance. Vivity approved in the US in February 2020, it’s the latest & greatest, but expensive. Do your research!!! My surgeon was an excellent doctor. Since I’m a airline pilot, he really got down & dirty with helping me understand my options. Alcon makes a trifocal, the Panoptix. Excellent lense as well. It will give 20/20 vision up close. Problem is as you shift visual aim points, you may have a moment of blurriness. And halos, stars, etc are common & may not go away. Their is no perfect IOL lens, it’s what’s important to you. I chose the Vivity for perfect vision across all ranges, except very close up. I can live with cheaters for those few moments in life. I’m typing this on my IPad 10 inches away, and the new eye doesn’t need cheaters, it’s just fine. 2nd Vivity goes in in a couple days. Looking forward to no glasses the rest of my life. Worth the investment to me. $3,300 US each lense.

      Thank you 🙏🏻 so much for your very informative video. I have cataracts, very bad nearsightedness, astigmatism and just learned I also have glaucoma to address during the cataract surgery too. Luckily it was caught in its early stages. Your video brought me closer to choosing the best lens replacement and the opthamologist is going to implant the Hydrus to address the glaucoma during the same surgery. I am currently using Lanaprost eye drops to bring the ocular pressure down since the recent diagnosis. I have been feeling a bit overwhelmed, but your video really helped!😊

    I like the clarity of your explanations and delivery. I'm just in the process of choosing the correct lens for cataract surgery. I have astigmatism and my goal is to see wthout glasses for most distances. I'm fine with readers for closer clarity. The surgeon suggests either Monofocal Toric SN6AT/SA6AT by ALCON AMO or B&L , or: Eyhance Toric DIU by AMO. I understand that the Eyhance Toric may have greater halo/ glare at night but since walking / functioning in daylight without glasses is my primary goal, I'm leaning toward it anyway for greater range of glasses free daylight vision. Any suggestions? I'm almost 78,

      I am glad you found the video helpful! Regarding the specific IOLs, the ophthalmologists/eye surgeons would be the ones to ask. Our care for patients as optometrists only go as far as referring the patient to the ophthalmologist. All the best on your cataract surgery! 👍🏻

    I had cataract surgery almost one month ago the lens to see near and far was recommended, for a charge of 2.400.00, my insurance did not help, so I had to take care of it alone. Now I don't need glasses, thank God I'm doing well, so I will recommend Envision eye care on SE 17th street in Ocala

      I am glad you found the video helpful! Regarding your questions:
      1) I would not recommend it.
      2) What you propose is called monovision. It works for some, but not for others. It's important to consult your ophthalmologist to see if this setup is suitable for you. In some cases, the patient can get a sense of how monovision will be like with contact lens trials.

    Thanks. It's the best video to understand the basics of implantation of the lens. My Mom has a cataract. Can you please give a little idea about the trifocal lens prices? Some of my relatives told me it takes $10,000 for this in Toronto and some other says that it takes $4000.I am so confused.😰

      Although the prices differ from clinic to clinic, I would say on average, the latter price of $4000 is closer to what you would expect for trifocal lenses. It can cost a bit more or less depending on the doctor or clinic.

    Very well explained. I ( 69 years old ). Have an appointment with the eye specialist 2morrow. Optometrist advised dense cataract in each eye. I guess with surgery they will do one eye at a time with a few weeks in between?

    Thank you for this video. I found the simulations very helpful. Especially the one at 4:08 for the monofocal lens. I wonder how anyone can live seeing things like this all the time. I couldn't.

      I had monofocal lenses implanted in my eyes. No regrets and my sight is just fine. Just need glasses to read small/fine print. Guess I'm the lucky one. Take care and stay well everyone.

      I can think of two common scenarios where a patient may need to wear glasses for distance viewing with the monofocal lens implants:
      (1) Patient chooses government-covered monofocal lenses (which corrects for the nearsightedness/farsightedness component of patient’s prescription), but significant astigmatism remain. This can be prevented if patient upgrades monofocal lens to toric design (explained in video).
      (2) Patient chooses monofocal lenses to focus at near (because they want to enjoy clear correction-free near vision rather than for distance).

    With a trifocals lens is the top of the lens for seeing distance, the middle of the lens for seeing objects at intermediate distance and the bottom of the lens for seeing up close like reading?

      Good question! The answer is 'No'. Etched onto the surface of the trifocal lens are concentric rings, which allow images at a variety of distances to focus on the retina. The simulations in the video demonstrate how your vision is with the trifocal lens design at any given moment, regardless of whether you look up, straight ahead or down.

    How long does the replaced lens last? Is it something that has to be replaced again after a certain time?

      The lens implant is designed to last and is not meant to be replaced in a given time.

    Hi doc is there a big dofeerence between hard lens with stitches and soft lens without stitches? Because my motger will undergo to cataract surgery and our doc give us 2 options the free hard lens or the expensive soft lens?

      Other than the considerations mentioned at 2:55 of the video, the obvious result of cataracts is blurrier and blurrier vision with time in the affected eye(s).

      That is definitely an option! In fact, if you naturally had nearsightedness that allowed you to read at near without glasses, it’s often better to set the focus to be at near because it’s what you’re used to already. You would expect to need to wear glasses for distance in that case (just as before).

    I now have double vision due to anesthesia needle hitting muscle. It sucks. Prism in prescription is necessary, it helps, for the most part, not perfect. Loss of depth perception. I have to use 3 pairs of glasses. Driving and distance, computer and close up which I can't afford right now after paying out of pocket for 1pair and deductibles. I don't like not seeing up close. Got dust in my eye the other day, I was helpless, kept trying to wash out my eye.

    I didn't know there are trifocal lens implants. I wear a bifocal eye glass and my intermediate vision is still blurry. But I can drive even at night...

    Thank you for the informative video. If money is not an issue, would you recommend monofocal or multifocal? Do I have to tilt my head to see the different portions of the multi focal lens like multi focal glasses? Also do the artificial lens feel natural or they feel like wearing glasses permanently?       The decision between monofocal and multi-focal lenses should be based on what’s important to you. You can review the pros and cons of each in the video.
      The multi-focal intraocular lenses do not work like multi-focal glasses. You will not need to tilt your head to see for different distances.

      @@mommydear Katie, if money is no object, I highly recommend the Alcon Vivity IOL multifocal lense. You can read about my experience up several post, where I replied to someone else who got the Vivity. Next surgery couple days away. After 2 weeks, no blurriness, halos, stars, problems focusing! It really is the latest & greatest. Approved in the US Feb 2020 by the FDA. I’m exceptionally happy with my decision.

      @@mariacabral3967 Maria, I’m extremely happy with the Alcon Vivity lens since the November 2021 surgery on both eyes. Still perfect vision. 20/20 distant, 20/15 mid, and 20/25 near. Do need very low power cheaters late at night with low light for reading cell or books. Colors are extremely vibrant. Sometimes a little bit of halo effect around red lights, or green lights at traffic lights late at night, easily dismissed. My surgeon was so impressed with my results, he said he may discontinue the multifocal lens in his toolbox. I had a 6 month follow up a couple of months ago. All test results were excellent. I, like you, did my research & due diligence. I fly jets for a living so we had to get it right. Thanks for asking.

    Doctor, Should you go for one eye surgery or both when you have cataract in one eye?

      If it is age-related cataracts, I lean towards recommending having both eyes done. This is because it’s only a matter of time before the other one ripens too. It also makes getting glasses post-surgery easier because both eyes will likely be balanced in prescription and stable for good.

    There needs to be a better way to do lens exchange why shouldn't people who have had cataracts years ago not be able to take advantage of new technology such as extended depth of focus many doctors refuse to do Lens Exchange

    I see you are an optometrist very familiar with them but they are not so familiar a lot of them with what eye disease I have which is keratoconus so I just had cataract surgery on both eyes but maybe I kind of lucked out with my new optometrist the old one retired. My new optometrist understands my condition he has keratoconus himself and believe me it can make an optometrist almost want to pull out their hair. I am not Canadian but I am American although my mom was Canadian.

    So it’s not necessary to be completely anesthetized? Local is adequate? My doctor told me I’d be given an IV.

      Aside from the local anesthetic to the eye, the anesthetist may give some mild sedative medication through an IV. The intention is to relax you and not to put you out. Hope that clarifies things for you!

      Upgrading a regular single vision lens implant to an aspheric or toric lens implant (to increase contrast sensitivity, enhance functional vision and to correct astigmatism) can cost a few hundred dollars. Multi-focal lens implants can cost a few thousand dollars. For exact pricing and lens implant options, it is best to consult your ophthalmologist.

      I have surgery next week. Insurance pays for single focus IOL. If you get lenses to see far, then you will need reading glasses. If you get lenses to see close, then you will need glasses to see far away.
      For serious money, $1800 per lens, a multi focal IOL can be done. You might not need glasses at all, but it might result in less sharp vision. Not recommended for a lot of nighttime driving, up close work...
      TORIC IOL - corrects for corneal astigmatism.

    Hi mommy my right eye has monofocal lense, what the best for me for the left, is mono or multi focal or another ? I've got minus 9 and Plus 2 before the surgery

      If you have a monofocal lens in the right eye, then usually you would have a monofocal lens in the left as well. As for what specific monofocal lens (i.e. what power) to choose for your left eye, it is best to consult your ophthalmologist.
