Now i have an idea what can cause the freewheel to fail.
The rachet mechanism has 2 springs overtime with use or riding through water , the springs either gets corroded or snap
In severe cases the freewheel can have a " stress crack " and dissentigrates, leaving only the 2nd half of the cogwheel on the hub
