That's interesting I took it as his wife/lover has become mentally ill and is no longer the same person so he's having sex with their ghost.
To me, it just sounds like he's talking about a toxic relationship with a woman he's physically addicted to. Not only does he compares her with a ghost (maybe because she haunts him even when she's not present) but also with a ghoul, a creature coming straight from your worst nightmares. In his voice, you can hear the fear and despair this woman inspires him, but he can't escape her because he's too afraid to defend himself (he closes his eyes when he pulls the trigger and only hopes he can kill her because he doesn't have the strength to fully assert himself and he eventually misses every attempt at escaping her grip) He also seems sexually addicted to her, and she's taking advantage of the fact that he has no one else but her to abuse him.
