Mostly QALY model but we also try not to neglect relatively "futile" patients who would benefit from needs-based. NICE uses a threshold of £20,000 to £30,000 per QALY gained for the cost-effectiveness of treatment, but also considers the societal benefit. There are also special funds and charities to pay for extremely expensive treatments with a few years of QALY gain e.g. end stage cancer treatments or rare genetic diseases. This means there is embarrassment for the patient and for the doctor, and the doctor can also exploit the patient sexually without her noticing and thinking that this is part of the examination.
