Does the fuel on the island get used when spawning a boat with fuel in it?
Also, similarly does it cost if I take my boat for a ride and return with all the medkit and welding torch used up?

Fuel from the fuel stores at each workbench is used to fill the tanks when a vehicle is spawned. You will get a warning message if you dont have enough fuel to fill the tanks completely.
If you use a consumable like a first aid kit the cost is not refunded when you return the vehicle. I think, Ive never checked to be honest :D

How do you do modular engine cooling?
When I do it they usually always overheat...

Flowrate seems to be the most important thing. If you place your radiator away from the engine I think you can get better performance. But a fluid pump in and another fluid pump out of the cooling manifold will do a lot more good than adding more and more radiators.
