I wonder how many endings there are since this was Ending 2. And I also wonder if the sister maybe collapsed on the rails which is why the brother said she wouldn't wake up no matter how long he waited. (So maybe they were there for a good minute and eventually a train came and because they were both near to the ground the conductor saw them too late)
I watched a couple other youtubers play and there's only 2 endings! This is the detailed one, the other is just the protagonist getting off the train after pressing the stop button (implication is that he died for real).

Nah, it’s one of those atmospheric, observational horror games. Like The Backrooms, Exit 8, Observation Duty series etc. This one just has more a limited narrative focus over scares.
