Doctor has to start with chief complaints, investigations,probable diagnosis, Rx, and then name of drugs to be given and their frequency and time of day, before or after meals, and then mention when to come back with test reports and for review.. with signature..
Side effects are not written they are told to the patents to look out for..
If it’s an emergency and patient has to be admitted.. the directions are given to the nurse..about the way the drugs are to be given etc and separate forms are given for investigations to the attendants..
In small clinics doctors themselves wrote the age name and sex, weight too sometimes

She got admission into Kashibai Navale medical college in 2007 using NT3 quota (Nomadic Tribe) and ahe produced a Non creamy layer certificate despite her father being a Class1 Officer.
She has used forged quota document to gain into MBBS under giverment category seat.
If she wouldnt have used the quota she wouldnt have gained admission into MBBS course under general category, hence she would never become a doctor.
Goverment should ensure even her MBBS degree must be also cancelled.
