Excessive Body Odor?

- Today’s topic of discussion is bromhidrosis commonly referred to as excessive body odor or BO.
I recently saw a young female who came in to get evaluation for excessive body odor. She was clearly very distressed about her condition and that was my motivation to make this video to help her and others like her who suffer from this condition.
We all have some body odor which is normal, however in certain individuals this can be excessive and very offensive and can lead to significant deterioration in quality of life.
Quite often the body order is detected by others however is not detected by the individual due to a condition called selective anosmia - which means the inability to perceive odour. So the individual may be completely unaware of their offensive body odor.
We have 2 types of sweat glands in our body,
Eccrine sweat glands are found all over the body and help to regulate body temperature by producing sweat.
Normally sweat is odorless, however sweat can get smelly if excessive amounts of bacteria are present on the skin.
Sometimes sweat can also secrete odors from a diet containing garlic, onions, alcohol or certain medications such as penicillin.
The Apocrine glands develop after puberty.
They are present in the underarms, breasts and groin area. These glands release pheromones also called as personal scent to attract the opposite sex. patients with bromhidrosis have larger and more numerous apocrine glands and also higher levels of dihydrotestosterone on the skin.
A familial link is also possible
This condition is more common in men since males have more pheromone secreting glands.
Both of these eccrine and apocrine glands are controlled by sympathetic nervous system and individuals may release excessive amounts of sweating and pheromones in response to anxiety and stress.
Rare metabolic disorders can also cause this condition
Tri-ethyl-aminuria is a rare inborn error of metabolism leading to a distinctive fishy odour
Certain diseases such as liver failure, kidney failure and chronic gout can also cause excessive BO
Cases have been reported in individual who had whole body laser hair removal.
Diagnosis
Is clinical and made by smelling the affected area. If no offensive odor is detected then a psychological workup is done to exclude schizophrenia
Certain blood tests are done to exclude liver failure, kidney problems, high uric acid.
Treatment
Eliminate odor causing foods from the diet such as garlic, onions, Curry, alcohol, medications such as penicillin.
If a surrounding skin infection is suspected then treatment with antibiotic cream is indicated
Improve hygiene and reduce bacteria by using antibacterial soap and rigorous washing of the affected area
Wear cotton clothes which will wick moisture away from the skin and prevent bacterial overgrowth
Do not wear the same clothes without washing
Reducing sweating is very important. This can be done using antiperspirants, Botox injection, iontophoresis among others. Please watch my video on hyperhidrosis which discusses this in detail.
Remove hair from the area by shaving or laser hair removal. This can help reduce the amount of sweating and may also help with body odor. There have been some reports of worsening of body odor after individual had whole body Laser hair removal, however this is a rare.
Liposuction curettage of sweat glands in severe cases
Microwave destruction of sweat glands, or surgical excision can be tried in severe cases not responding to traditional treatments.

There’s a guy that works in a shop that I go to. I always know if he’s in because I can smell him before I see him. I know this doesn’t sound good but he’s such a great guy and I usually have a nice chat with him. I was always curious why he didn’t sort out this body odour issue. But after watching this video it might be because he can’t smell it himself.

He probably CAN smell himself. He just doesnt know how to get rid of it.

What if antiperspirant and deodorant makes you smell worst? I can't even use baking soda paste it all makes me smell worst I've tried 10 different even certain dry and drysol .
Thank you for your comment. This must be so frustrating for you. Have you been tested for other diseases that could affect the smell of sweat produced? At a minimum, I would recommend getting lab testing done for thyroid, uric acid, and a metabolic panel to evaluate for liver or kidney problems. Some people has a strong aloe vera type bodyoder ,and its completely based on gentic but can it infected or transmit a person if we wear a same shirt of the desease person..
Thank you for your comment. What we eat has a strong correlation to body odor, for example, a diet high in onions and garlic will alter body odor. I am not aware of aloe vera type smell causation. Just wearing clothing from a person who has bromhidrosis (excessive BO) will not transmit the disease
