By a wrong question you could mean the wrong answer to a practical problem. I.e. the PhD-student is losing sight of the hypothesis he has initially proposed. (Uninterchangeably) I assume you mean that not enough contemplation is taken on what questions should be spent time on in the first place. Indeed, I remember having collected my Introduction info from my masters thesis right at the end, just to fill in the fluff (what are biomolecules, what can they tell us...). Not really review paper material, and it usually helps to find context when formulating your questions (it keeps essays shorter as well!). However, in the case of basic research he may just want to study a phenomenon and describe causes of it as an introductory review to a topic. Though I have not seen the paper, I suppose 1) the paper to publish this article is too high-rank and 2) the questions are too simple to answer in such a lengthy manner, which is why you rejected it. On a sidenote, nor is it that purely academic research is inherently bad, it really depends on the field of study and question proposed. But I did understand your takeaway: impactful research is grounded on the reality of said field and its professionals

What I think is that researchers should spend good amount of time to think about their question. Discuss the question and get feedback from others. In specific disciplines it makes sense to talk to practitioners and get their views. In the end we should be solving problems that are not solved.
