You have learn database for that friend. you can understand it a database instance. This concept was introduced from oracle database 12 onwards where you can have multiple instances on same databse each instance called pdb. You can use each pdb for different applications or different customers as a separate database
There is no single answer to this. You have to look for the exceptions in logs and based on that you have to take necessary action based on the impacted system

data sources targets will be for cluster or individual Managed server ?
The target of the data source should be the same as of the application going to use it.

Managed server is going to failed state every time what could be the issue how can we troubleshoot ?
There could be multiple reasons....memory issue, cpu issue, file system issue, deployed application issue, network issue, database issue etc etc
