    EXCUSE the spelling
    Acne:
    1. caused by increase in oil production (oil blocks pores)-> "stickness" skin cell cohesiveness-> bacterial overgrowth -> inflammation
    2. increased oil production aka sebum
    3. stopping oil production is difficult. But you can keep oil from clogging your pores
    4. stop oil production
    1. niacinamide and green tree extract can decrease oil production for some.
    2. ORAL prescription can also reduce oil production.
    3. Salicylic acid can help get rid of oil but not decrease
    4. *Don't over scrub your skin. harsh exfoliants. irritation = produce more oil. (astringent, alcohol) - not good.
    5. decrease pore obstruction. pore obstruction = causes bacteria to build up.
    1. retinoid - flakiness and "purging" expected for a few weeks, a good sign because that is what this vitamin is supposed to be doing.
    2. aha, glycolic acid and other bha's
    6. decrease bacterial overgrowth
    1. benzol peroxide
    2. syclic acid, glycolic acid, azelaic acid will also have some antibacterial effects
    7. decrease inflammation
    1. other things above will get it done. But for a specific targets to inflammation do below.
    2. nicinamiade, retinoids
    8. *retinoids target 3 out of the 4 of the acne process above.

    Like another commenter said, please talk about the role SUGAR CONSUMPTION plays in acne! I have been under the treatment of a dermatologist from age 10, due to terrible cystic acne that runs in my family. I've done everything...accutane, doxycycline, retin-a, and so many more. Then I read somewhere in a book that cutting down on sugar would help. Within a day or two of cutting out sweets, my acne situation improved more than it ever had with any of the prescriptions. Had I known this earlier it would have saved me from the years of embarassing skin problems and the terrible scarring I now have on my face. Why don't more dermatologists talk about this?

      the same experience. cutting dairy and sugar I had an incredible improvement after years of adulthood with acne. Now it only comes out if I'm extremely nervous. It has to do with these "foods" being highly inflammatory. dermatologists don't talk about it because current medicine still treats the body as if it were separate parts of a whole. I add to whoever reads this that in addition to eliminating these foods I no longer consume gluten or processed foods. I have multiple food sensitivities that's why I gave it up, but they are also inflammatory foods. In any case, the final improvement was removing sugar and dairy products.

      American medicine is very analytical and high-tech. Each specialty will address a set of specific problems. Solutions were created by researchers and the industry. Doctors master it and it is very efficient. So each specialty takes care of his part of the body or system as taught in medical school, with no place for a holistic approach, and no willingness for war among specialists. And why do Americans want to be on a no dairy, no sugar, and maybe no wheat diet and take probiotics? Exit the pizza! And all fast food should be banned, there is sugar quite most everywhere. So no life for a teenager and his parent.
      On the other hand, inflammation, which is an immunologic reaction, is not very well understood, just look at COVID treatment at the beginning of the pandemic and the emptiness of a solution for Long COVID. This systemic reaction needs a broad approach.

      @@epicgoblin3564 it could be helpful to look into avoiding "inflammatory foods" or trying out an "anti-inflammatory diet". Another possibly helpful way to go about it might be trying Whole30, since it's geared toward cutting out inflammatory foods for 30 days and slowly introducing foods back to see what specific to you has good or bad reactions. Everyone is different, so this might be the way to go. Good luck!

    Hi , I’m from Germany and here going to a dermatologist is not very common.
    It is also very hard to get an appointment and if you get one the dermatologists can’t really help you a lot .
    I’m so thankful that there are people like you who want to help others and are not all about the money.

    I use Benzoyl Peroxide under my arms in the shower because bacterial waste products is the primary cause of body odor (per Dr. Dray) and now my underarms barely have any smell! Thanks for the great tips for everyone. ❤️

      @@Deepak-sn7oc you have to 1st understand what causes acne.....for some people it's hormonal, for me it's my stomach heat n excess oil production in face......no amount of skincare product can cure that, so I incorporated some changes in diet like eating cucumber daily, taking coconut water, curd, avoiding packet n oily foods etc n not to forget wash ur face daily at nit before going to bed.....I hope it helps bro :)

    I’ve had acne for over 10 years, and the only thing that has ever helped my oil production and my acne has been changing my diet, including not eating high glycemic foods and not eating dairy. Once I changed my diet, it took me about three months to notice a difference in my skin, but it changed everything. It’s so worth it. Not even a Accutane helped me as much as changing my diet did

    I'm so excited that you were in Hyram's video and I found you! I am a science oriented person (just took my MCAT today) and am excited to learn from you. I just started my second round of Accutane today, 8.5 years after my first round which cured my acne for 5 years, and would love a video about tips and products to minimize side effects. Accutane was the only thing that could fix my acne the first time and now that I have mild acne again and have tried everything topical again I'm happy to get back to clear skin.

      Hi! Thank you for heading over here! The most common side effect we see with accutane is dryness. Key is to moisturize with creams (ceraVe moisturizing cream) and thick ointments like vaseline! If side effects are really bad, you can always discuss decreasing your dose with your dermatologist but it does usually get better after the first 2 months.

    Thank you for the wonderful tips. I would've loved it more had you discussed the relationship between excessive sugar/carb intake and insulin spikes leading to excess testosterone and therefore an increase in oil production. I saw so many dermatologists as a teenager and not one told me about this cascade, which ultimately would've allowed me to treat the root cause of my acne!
    Great info for acne products but why don't we focus more on prevention from the source?

      @@elliewaters6040 yeah hopefully indeed it’s so bad I actually never take my mask of and don’t even post on social media or ever put a picture of me as my profile pic as like u have here I don’t even take pictures of myself on my phone that no one else will look at EVER the only pictures i ever took that was once and it was with a mask on and hood up I’m definitely too insecure about my looks and to make things worse I have terrible acne it’s so stressful and actually makes me want to kms

      @@abdulwadood.8 I feel you, it gets better ❤️ I had to force myself to take progress photos and it was so hard not to use a filter. I smiled even though it made me want to cry. This was last July and I can now say I have been acne free for 7 months. It does get better, I recommend going to your dr and maybe getting a blood test. :)
      In the end I had to remove my IUD, go on birth control, and use tretinoin which has helped tremendously with the healing of my acne scars

    thanks so much for this video! i've noticed that using retinol one night and benzoyl peroxide another has helped a lot but i'm still breaking out on the lower cheek and jaw area. i'm thinking it's hormonal (25 y/o female) and will be seeing a dermatologist soon, but until then i'd love to hear more about the different types of oral treatments just so i can go into my appointment a bit more informed

    Thank you so much for the video and the great info! I’m actually learning a lot. Now, I suffer from acne and I am wondering how to combine retinoid, salicylic acid and niacinamide into a good skincare routine.

      I've acne too. I've used majority of the actives they talk about, at some time.
      Initially, I used Benzoyl Peroxide face wash, a clindamycin+adaplene gel, and an anti-biotic.
      Currently, I'm using a Salicylic acid face wash, use adapalene(at night) for scar treatment. Occasionally, when I do get a pimple or two, I use Clindamycin. Hope it can be of use.
      Consulting your derm would be the best step.

    THE GLUTEN FREE DIET! After being diagnosed in 2005 with Celiac Disease and having to go strict Gluten Free my lifetime (55 years of acne) finally disappeared. If I had only known that I had to eliminate this from my diet I would have done so years earlier and never developed Celiac. Being free of the gastrointestinal symptoms was great but the clear skin was an unexpected bonus. Hope this helps someone else. Here it is 2023 and skin still acne free.

    My acne is tricky. It appears 1 by 1 by 1 after 1 healed then gets another 1. Seldom gets pimples 2 or 3 at the same time. 😂

    This has been so helpful as I struggle with comidonal acne, fungal acne, and hormonal acne. Just found out I have pcos and that’s why I have such oily skin(: sulfur has been the biggest game player for me!

    I'm on tretinoin I use it every 3 days, I'm gonna give it 4 months to see a difference, at the moment, no difference after 3 weeks.

    Great video! I was disappointed to hear that diet was not mentioned as things like dairy and refined sugar have been directly linked to acne production. I have seen 80% of my skin issues disappear by eliminating these consumables. This seems like targeting skin issues at the root. I would love to see a video on diet in relation to skin from you!

      SO TRUE!!! Cutting sugar out of my diet did so much more for my acne than Accutane, Retin-A, doxycycline, and all the other stuff that my dermatologist tried.

    Thank you so much for sharing your knowledge. I’ve suffered from mild/moderate acne since a teenager. Now at age 31, it still hasn’t cleared. It has really impacted my self esteem.     If I’m consistently treating my skin with acne fighting ingredients from my dermatologist, nightly, then why would I continue to break out? And we’re talking years. Adult years, at that.

      It depends what kind of treatment your dermatologist has prescribed you… If it is all topical treatments, you may want to ask your doctor about oral antibiotics or other alternatives. Another option would be looking at external factors such as stress and how that plays a role in hormonal acne. Additionally, you may have some sort of chronic inflammation due to an undiagnosed food intolerance. Long story short, there are a lot of factors that play into acne. You have to consider external factors such as bacteria and internal factors such as hormonal imbalances. If you not happy with your progress after actually remaining consistent with your prescribed treatment plan, and have explained this to your dermatologist, you may want to consider finding another dermatologist. Sometimes a new perspective can be really beneficial, and help you find a provider who is willing to work with you as you advocate for your skin health. I hope this helps!

      @@alex-mj8pn yep, I tried oral antibiotics. I’ve tried different topicals. Different birth controls, discussed food intolerances but that’s something I wanted to do last. So I switched my dermatologist, actually to these guys! And so far, I’m having really great results. Still the hormonal breakouts but I suppose that’s just the cards I’ve been dealt. Actually seeing them tomorrow for my follow up.

    For anyone reading this and dealing with acne from very long time believe me scarring can happen with prescription strength treatment too due to reoccurring of acne on same spot again and again.

    Watching for myself lol I told my oldest who got my acne gene I said "you have a dermatologists I didn't back when I was young if you were ugly you stayed ugly" 🤷🏽‍♀️lol that's how I got him to stick to his tretinoin his topical antifungal and oral antibiotics. My whole side of the family has scars from severe acne. Now he has great skin and no scars 🤞let's hope he keeps up with it. I say do you want to be like me your Tia's and Grandpa? He washes his face real quick

    Turmeric is a holy grail in South Asia, a quick turmeric and sandalwood powder mask is used for brightening for a small period of time as well. Turmeric is used in Ayurveda in so many ways, I really hope a good formula comes up. Its a good option.

    I did 2 cycles of Accutane 60 mg for 5 months, then 80 mg for 5 months (plus all the bloodwork to check my liver function). It helped.
    What made my skin gorgeous was spironolactone: as gorgeous as it was on Accutane with none of the dryness, none of the bloodwork, none of the liver toxicity.

      Do you mind me asking if you had kids after your two cycles of Accutane? I have been wanting to ask women. I am supposed to start the end of this month and I already have fertility issues. I know that the medical community “says “ that it doesn’t cause issues with future fertility but we all know how the medical community will often downplay side effects or not mention them all together. Can I also ask you age?

      I have no idea if any of this will help you and your situation, but it's a synopsis of the many years of research on these topics that I've done, along with my subjective experience of what has and has not worked for my body.
      Lastly, one of the most important lessons I learned from the Accutane-birth control pill-debacle that caused candida albicans to run rampant and destroy my intestinal microbiome and cause me two years of income (in Manhattan, nonetheless!) and two years of my youth (age24-26), plus costs me THOUSANDS of dollars in prescription medication and blood work (and this is NOT including the medical expenses over the 2 years it took me to recover). No matter how much education your physicans have, no matter what prestigious universities they've attended, no matter if they've been your physician for many years, YOU must do all your own research and make certain that they are staying current on reading the most recent medical literature.
      If you do not educate yourself well and thoroughly, and you do not become your own medical advocate, there is a strong likelihood that you will go undiagnosed or misdiagnosed. Even with all my education, both self-taught and academic (UC Berkeley), "specialists" has not only misdiagnosed me several times, but also NOT diagnosed me multiple times. This has been a very costly lesson, and it has taken me MULTIPLE health incidents to learn it thoroughly. I take my physicians, specialists, and surgeons to task...in their area of specialty and I do not back down. Most of them respect this, and are not threatened by it. If they are threatened by it, RUN do not walk away: RUN and don't look back or 2nd guess yourself.
      I have known women who had PCOS, endometriosis, and full blown menopause who have all been told they are infertile and, years down the road (including the 50 year old woman who had been in menopause for 3 years...she thought) ended up getting pregnant once they quit stressing about and accepted the "fact" that they were infertile. I have also known women who have never gotten pregnant, and there is no known reason. They never had an unplanned pregnancy, nor a single miscarriage. One of those women was married to a narcissist for several years and he made her believe SHE was the problem. He even claimed to have had his sperm tested. 8-9 years went by and, a new fertility specialist wanted to meet both of them to discuss options and, in that meeting, asked the PA to take the husband over to the clinic to obtain a new semen specimen they could retest. She stated that he suddenly turned white, as if he might get sick or faint. Turns out, he had been treated for some kind of juvenile cancer and never told her, sat by and watched her stress over it for year, go through treatments, and here they were in the office of the IVF specialist. One has to wonder if he was actually going to PAY for her to go through IVF or if he was finally going to tell her, had the specialist not requested his own semen specimen to check for current motility, etc. Needless to say, they divorced over that years-long deception (and I'm not at all suggesting this pertains to your situation...just giving you a run down of various wild stories I've heard over many years and decades).
      Each and every year that passes (I'm now 50), I'm learning more and more how unreliable some of this testing is. When I was 46, I went in for a yearly pap, etc. and they were testing my thyroid hormones, as I was particularly thin (body of a 23-24 year old), but I was also spending a good chunk of my day walking to/from the train, around the city, etc. In fact, I was walking so much that my feet would hurt at night. Thus, the slender frame made sense, based on my activity level. In the follow up appointment, I was told I was no longer able to conceive and, I was both relieved and a little saddened to be moving into that stage of life. One week later, I was pregnant again, 6 months before my 47th birthday. I could feel it within a day or two. I really didn't think that was possible, not just because of my age, but because they had just drawn blood to do a full thyroid panel. I was floored, but when my period never started, I knew those early feelings were right. So, keep that in mind. I have been perpetually *astounded* at the narcissistic hubris in some (not all) medical professionals. Conversely, I've been equally amazed by the humble brilliance and professional integrity of those medical professionals who are regarded as the top of their particular medical specialty. It's almost as if their depth and breadth of knowledge, along with decades in their field of specialty, have taught them to be humble, stay intellectually curious, keep up to date on reading the latest journal articles, and to LISTEN to their patients with the understand the many (not all) people know their own body and what feel "normative" better than the physician, even when they don't know why they feel a certain way.
      Keep yourself in the best physical and psychological health possible, including very low stress, relaxation, and good sleep. Find yourself the expert in the field, and find them by asking each professional you've been to whom they would send their best friend, sister, or daughter if she were in your position. Ask multiple professionals, and when you hear one docs name, repeatedly, ask for a reference to see that professional for an assessment.
      Stress is the WORST thing for pregnancy. If you think about it, from a survival-biological level, it makes perfect sense. Offspring has a far less likelihood of surviving if their mother is stressed out, or if she is worried about finances, or worried about her partner leaving her. These are all real fears females have shared with me. Like wise, many women who have had early miscarriages, or lost pregnancies, had these events occur either simultaneously or right after an extremely stressful event (psychological or emotional stress). Being too thin makes conceiving difficult, as does being over weight. Oddly enough, both times I've gotten pregnant, I've probably been at some of the healthiest times of my life, both physically and mentally, and my skin has been free of all break outs (which indicates a strong, well-functioning immune system and microbiome). That might be coincidental, but looking back on it now, I'm not sure that it was pure coincidence. Both occurred when I was relaxed and not "stressed" about getting pregnant. My older sis couldn't get pregnant the first 6 months she tired, but she was OVERLY stressed about trying and it not happening. Our mother told her to relax and stop trying so feverishly. She got pregnant shortly thereafter. However, she also lost some weight and began exercising more regularly and eating better, which helped her stress level go down. She never had acne, though, and she never took Accutane like I did.
      It has been a WILDLY stressful 2 years. Hormonal acne flares up when your hormones are disrupted, and nothing disrupts hormones quite like life-threatening stress and fear on top of the already hormonal ups and downs of the female ovulation/menstruation cycle. Plus, masks have brought out so much acne in adults, including my own (after years of not having any...I'm almost 51). It's almost exclusively around the mask line where it fits snugly to my face.
      I wish you well in going forward to become your own well-informed health advocate! I also wish you a healthy happy pregnancy and baby in the future!

    Thank you gusy, you helped me a lot! I went to a dermatologist with my acne, he gave me Effezel-gel ( or Epiduo as you know ). From this video i get the information that this Effezel treats 3/4 of my problems, just only the oil-production is not treated, so bc of the video i knew i need it as well, so i got a mousturizer with niacinamide, and my skin is perfectly clear and glowy! Thank you again! 🤗❤️

    Thank you for this informative video! It would have been helpful if you touched on prevention. I’ve heard that diet high in carbs can play a role in increased acne through increased insulin which increases androgens which in turn increases acne production. Prevention from a dermatologist perspective would be helpful. For what would be the use of these products you highly recommend for treating the acne when you don’t treat the root cause?
