Thanks for the feedback madam. I personally use out of plane approach for interscalene. Actually it is something in between inplane and out of plane. If going for in plane approach, no special consideration for interscalene block. We have to come from the posterior side and direct towards the anterior part. Insert the needle just posterior to the probe. And then follow routine technique for visualisation. Hope it is clear what I explained. Thank you.

Sir...when the probe is not straight but tilted as in brachial plexus block...at that time how should be the needle angulation?...can u please explain..
Whatever the probe position is - needle should come in line with the ultrasound beam and at an angle less than 45 degrees which is easily possible in brachial plexus block.
For supraclavicular block - try keeping a pillow patient shoulder and with neck in a slight hanging like position to expose the supraclavicular region.
It can be simplified like this. For example you are giving the median nerve block at elbow level. Once you scan and identify the nerve, keep it in the center of the screen and then see how deep it is from skin. How much ever deep it is, take 1.5 times that depth as the point for needle insertion from the centre of the probe. For example, if median nerve is seen at a depth of 1 cm, then a point 1.5 cm (1.5 times 1) away from the centre point of the probe is where you should insert the needle.
