I think they had death ward, which gives you back all your actions that turn when it activates (which is why they went in at very low health) so that means 4 attacks from action surge, another 2 from bonus actions, then after death ward you get both actions and bonus actions back for another 6 attacks

​@@aslo7365this is 12 fighter, so he get 3 attack for 1 action, with surge that become 6 + 2 bonus = 8 and cause he died but revived, it reset so he got another 8 , in total 12 action, and 4 bonus action

@@aslo7365close but not quite right. it is survival instinct part of illithid ability not death ward. so basically this build work on any class that don't have death ward
